As an integral member of the interdisciplinary treatment team, the Inpatient Social Worker provides a comprehensive range of direct and indirect services to patients and their families. Responsibilities: • Conduct comprehensive psychosocial assessments for all assigned patients, including history, strengths, risk factors, and social determinants of health., • Provide short-term therapeutic interventions including supportive counseling, crisis intervention, psychoeducation, individual therapy, family therapy, and group treatment, • Participate actively in treatment planning with the interdisciplinary team, contributing clinical insights and advocating for patient needs., • Develop and implement individualized discharge plans in collaboration with the patient, family, and care team., • Coordinate referrals and linkages to community services including outpatient mental health providers, housing, substance use treatment, higher levels of care and social support services., • Communicate regularly with outside agencies and post-discharge providers to ensure continuity of care., • Performs and documents weekly assessments throughout patient's stay., • Serve as a liaison between the patient, family, treatment team, and external agencies., • Facilitate family meetings and treatment conferences as needed., • Collaborate with legal, protective, and government agencies in cases involving guardianship, child/adult protective services, or court-mandated treatment., • Participate in departmental meetings, case conferences, and ongoing professional development., • Adhere to ethical and professional standards in accordance with NASW guidelines and hospital policies.
