While Registered Nurses oversee clinical assessment and treatment, Patient Care Technicians (PCTs) are often the ones patients see, hear, and connect with most. You are the calm presence in the home - the person who explains, reassures, observes, and ensures patients truly understand whats happening. This is not a clinic-based role. It is a field-based, patient-facing position supporting dialysis patients in their homes within a defined territory. This job is designed for someone who brings empathy, emotional intelligence, and follow-through into every home they enter. Technical skills can be taught. We are seeking individuals who: • Communicates clearly and checks for understanding, not just delivery of information, • Pays attention to body language, tone, and engagement and adjusts accordingly, • Can read the room and recognize confusion, anxiety, or hesitation, • Makes patients feel heard, respected, and cared for like family, • Demonstrates genuine empathy, patience, and compassion, • Is dependable, organized, and follows through consistently, • Takes responsibility seriously and understands the importance of reliability, • Is affable, approachable, and able to build trust quickly with patients and caregivers., • Works well with nursing and operations teamsKey Responsibilities Patient Education & Support • Reinforce dialysis education provided by nursing staff, • Educate patients and caregivers using approved materials and protocols, • Confirm comprehension using teach-back and observation, • Provide compassionate, patient-centered support in the home, • Identify and communicate concerns or changes to supervising RNRNTechnical & Dialysis Support, • Travel between patient homes within assigned territory, • Support dialysis equipment setup and breakdown (within training/scope), • Perform blood draws and specimen collection per protocol, • Assist with troubleshooting equipment issues as trainedDocumentation & Collaboration, • Document patient interactions and education in the EMR, • Communicate timely updates to clinical leadership, • Support care coordination and continuity, • Maintain compliance with all policies and proceduresQualifications Required • Patient Minimum 1.5 years of prior dialysis experience, • Documented patient education responsibilities as part of prior role (examples include EMR documentation, care plans, education notes, patient onboarding, modality education, or training records), • High School Diploma or GED, • Valid Florida Drivers License, • Ability to travel between patient homes throughout the workday, • High level of professionalism and customer service orientation, • Experience using Electronic Medical Records (EMR) systems, • Comfortable documenting patient education and interactions, • Proficient with basic computer systems (Outlook, Word; Excel helpful), • Strong written and verbal communication skillsPreferred, • Experience in home dialysis (HHD or PD), • Experience educating patients in dialysis, nephrology, or chronic disease, • Prior field-based or in-home healthcare experience, • Familiarity with DaVita, Fresenius (FMC), or similar dialysis environments, • Bilingual (English/Spanish or English/Creole) a plusWhy This Role Is Different, • One assigned territory - no floating, • Patient-first, relationship-driven care model, • Strong RN partnership and leadership support, • Opportunity to make a meaningful impact in patients lives, • Field autonomy with accountability and structureComprehensive Benefits Package Health & Wellness • Medical, Dental, and Vision Insurance, • Paid Time Off (PTO), • Paid HolidaysFinancial Security, • 401(k) Retirement Savings Plan, • Hourly pay: $18.00 - $20.00 per hour, based on experience, • Mileage reimbursement at the IRS-approved rate for all patient home visitsWork-Life Balance& Field Structure, • Field-based role with regular patient home visits throughout the workday, • One primary territory to support predictable scheduling, • Supportive leadership and team-based environment, • Mission-driven home dialysis organization focused on continuity of careReady to Apply?
