SALARY: The average starting salary for this position will fall in the range of $65,000 - $78,000. Where candidates fall in this range will be based on skill and experience level. $2,500 Sign-on Bonus for first-time employees with MFS! ($1,250 on the first paycheck and $1,250 after 6 months of employment). ESSENTIAL JOB FUNCTIONS: • Assess patients need for a physical exam and conduct health assessments, including, but not limited to, communicable diseases, tobacco use, vital signs, medication reconciliation, and screenings for medical and behavioral health conditions, • Coordinate medical care, including providing referrals to community medical providers, scheduling appointments, and liaising with external providers to obtain patients' records, • Conduct metabolic monitoring (i.e. blood pressure, weight and body mass index), • Obtain and ensure processing of lab work for patients drawn both internally (if applicable) and externally, • Administer medications, including injectable psychotropic medications, following prescribed guidelines and safety protocols, • Provide basic first aid and manage medical emergencies and crisis situations appropriately, • Monitor and document patient responses to treatment and medications, reporting concerns to the prescribing provider and treatment team, • Provide patient education/counseling, both individually and in groups, regarding medications, chronic conditions, behavioral health diagnoses healthy eating, exercise and fitness, family planning, safe sex practices, harm reduction strategies for substance use, and general wellness, • Engage in outreach and follow-up to promote patient engagement and adherence to treatment plans, • Support crisis intervention efforts, including de-escalation and collaboration with emergency services when necessary, • Coordinate with pharmacies to ensure patients have timely access to prescribed treatments, • Perform and submit prior authorizations for medications, procedures, and other treatment services, including reviewing denials and obtaining additional information that could alter the initial decision, • Collaborate with psychiatric providers, therapists, case managers, and other healthcare providers to coordinate comprehensive care, • Maintain accurate and timely documentation in the electronic health record (EHR), • Participate in care team meetings, case consultations, and treatment planning discussions, • Ensure compliance with state and federal regulations, as well as CCBHC quality and reporting standards, • Graduate of an accredited nursing program Associate's degree in nursing, required., • Current RN license in the State of Illinois (or ability to obtain), preferred., • Current CPR/BLS Certification issued by American Heart Association, required., • Proficiency in documentation and electronic health records., • Strong interpersonal and communication skills., • Ability to handle multiple clinical responsibilities with attention to detail., • Training and experience working with people of all ages in behavioral health, mental health, or substance use treatment settings, preferred., • Willingness to work collaboratively in a multidisciplinary team model, • Bilingual in English/Spanish, preferred., • Certification in Psychiatric Nursing, preferred.
