General Summary of Position Provides both primary and relief nutrition management services on all assigned units, including the ICU. Participates interdisciplinary patient care rounds, performance improvement initiatives, clinical pathway development and committees. Primary Duties and Responsibilities • Provides nutrition management services to include patient screening, nutrition assessment and nutrition care-planning. Demonstrates knowledge of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient's requirements relative to his or her age-specific needs. General Summary of Position Provides patient care for transplant patients following established standards and practices. Functions to advance the clinical mission and goals of the Georgetown Transplant Institute through collaborative relationships within the Medical Center. These functions are performed in accordance with all applicable laws and regulations and Georgetown University Hospital's philosophy, policies, procedures and standards. Primary Duties and Responsibilities • Handles intake calls from potential transplant candidates and/or referring physicians. Assess patients for placement on the transplant waiting list. Coordinates evaluation with the patient/family and appropriate members of hospital, the transplant team, the patient's primary care physician and cardiologist, and clinical consultants., • Greets patients and prepares them for physician evaluations. Screens patients for appropriate information. Job Description INTRODUCTION Unity Health Care has launched a Food Pharmacy Program, a collaborative effort with Capitol Area Food Bank, to combat food insecurity for senior patients aged 65 years of age and older. The Food Pharmacy will target seniors who have hypertension, diabetes and/or obesity at Unity’s East of the River health center site. Under the supervision of the Medical Director, the Nutritionist will provide patient nutrition education and consultation. Also, the nutritionist will determine and prioritize the nutritional risks of clients based on established priority factors. As part of Unity’s responsibilities, they are required to provide evaluation and assessment of medical diets provided by the correctional food vendor. They also have a small number of patients with specific conditions who need nutrition assessment and counseling. A part-time registered dietician nutritionist is needed to fulfill these functions for approximately 2 hours/week or 8 hours/month. MAJOR DUTIES Summary of Responsibilities 1.
