Requirements: Key Areas of Responsibilities Community Engagement & Partnerships • Build and maintain relationships with community organizations, schools, nonprofits, faith-based groups, and local stakeholders., • Represent the organization at community meetings, outreach events, and community functions., • Identify new partnership opportunities to expand the program reach, • Serve as a point of contact for outreach requests and community partnerships. Administrative & Programmatic Support • Manage outreach calendars, event logistics, materials, and post-event follow-up., • Support tracking and communication of partnerships., • Assist with recruitment and onboarding of caregivers, older adults and program participants., • Attend select community events to professionally represent the organization., • Support data tracking and reporting related to outreach and engagement activities., • Assist in coordination of caregiver and social connection programs. Caregiver & Social Connection Program Support • Conduct quarterly telephonic outreach to enrolled older adults to complete required assessments and wellness check-ins., • Re-engage inactive participants to support retention and promote ongoing participation., • Assist with recruitment and onboarding of caregivers and older adults into social connection programming., • Maintain accurate tracking of assessments, engagement activities, and participation metrics to ensure grant compliance and reporting requirements, • Coordinate logistics for caregiver workshops, Memory Café gatherings, and social connection events., • Support preparation of monthly and quarterly reports related to CVS programming outcomes and engagement data., • Provide support for online/virtual events as needed., • Maintain and update program databases to track attendance, quarterly assessments, and gift card disbursements, including monitoring when funds need loading to participant cards. Program Promotion & Outreach • Promote programs and services through community outreach, events, and partner networks., • Assist in developing outreach materials such as flyers, presentations, and monthly newsletters., • Coordinate tabling events, workshops, orientations, and community presentations. Event Planning & Coordination • Plan and coordinate community engagement events, including scheduling, logistics, communication, and event materials, and follow-up., • Provide technical assistance and training support to Faith-Based Initiative program participants, • Ensure seamless execution of outreach events and positive participant experiences Data Tracking & Reporting • Maintain accurate documentation of outreach efforts, partner interactions, participant engagement and event attendance., • Prepare monthly and quarterly reports summarizing progress, outcomes, and key metrics., • Ensure compliance with grant and program requirements Internal Collaboration • Collaborate with internal teams to stay informed about programs and initiatives., • Provide community insights to help guide program improvement and planning. Qualification Education and Experience • High School Diploma or GED Equivalent required, • Associate or Bachelor’s degree in Communications, Public Relations, Social Work, Community Development, or a related field preferred but not required, • 1–3 years of experience in community outreach, engagement, program support, or related work., • Experience working with diverse communities and populations is highly required. Skills and Competencies • Strong interpersonal and relationship-building skills., • Excellent verbal and written communication abilities., • Strong organizational skills with the ability to manage scheduling and event logistics., • Confidence in public speaking and community representation., • Ability to track and organize data effectively., • Comfortable working independently and in a team environment., • Proficiency with MS Office; experience with CRM (e.g., Salesforce) or tracking systems is a plus. Physical Requirements • Ability to locally travel frequently throughout the Greater South Side of Chicago and assigned territories for community outreach, partner meetings, and events., • Occasional evening or weekend required for outreach activities, • Must have reliable transportation and a valid driver’s license (if driving is required)., • Ability to stand and walk for extended periods during community events, health fairs, canvassing activities, and partner engagements., • Ability to lift, carry, and transport outreach materials and event equipment up to 30 pounds (e.g., banners, folding tables, promotional materials, display items)., • Ability to set up and break down event booths, including bending, reaching, kneeling, and occasionally light physical setup., • Ability to work outdoors in varied weather conditions during community activations and events., • Ability to sit for extended periods when entering data, preparing reports, or conducting follow-up outreach via phone and computer., • Ability to use standard office and mobile technology, including laptops, tablets, smartphones, and CRM systems (Salesforce)., • Ability to communicate clearly and effectively in person and over the phone with community members, partners, and prospective members.
