Job DescriptionWhat You’ll Be Doing You will work collaboratively with the Executive Office of Health and Human Services (EOHHS), Department of Human Services (DHS), and/or The Department of Behavioral Health, Developmental Disabilities & Hospitals (BHDDH) to provide case management services to participants who are part of the Conflict Free Case Management Model. The Case Manager provides in-home visits, monthly outreach, person-centered plan development, oversite, referrals, and monitoring. The case manager may assist with applications and referrals to community and state funded resources.
