About the job: Schedule: Part-time Mondays & Fridays w/ 4 additional hours rotating on-call Pay: $56.00-$65.00 Hourly The Nurse Practitioner under the supervision of the Medical Director provides primary care to participants. Performs physical assessments of new PACE Enrollees, semi-annual reassessments of participants and develops and implements appropriate plans of care to Senior Community Care program participants. Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT). Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and Senior Community Care standards. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Licensed as Registered Nurse (R.N.) in the state of employment, • Graduate of a school of professional nursing required., • Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing, • A minimum of one year’s experience in working with the frail and elderly population required., • Shall have either training or related experience in the job assigned. Preferred Qualifications: • Certified as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority preferred., • Two (2) years’ experience as a nurse practitioner in a geriatric setting desirable. Essentials: • Responsible for oversight of residents of VOANS Skilled Nursing Facilities., • Evaluate residents of VOANS Skilled Nursing Facilities., • Provide medical care and treatment for residents of VOANS Skilled Nursing Facilities., • Coordinate care of residents with Primary Care Physicians and Medical Directors of VOANS Skilled Nursing Facilities., • Performs in person comprehensive history and physical on new Senior Community Care participants. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant., • Conducts an in person reassessment semiannually and as needed., • Integrates the primary care treatment plan into the overall plan of care developed by the Interdisciplinary Team. Interacts with team members to meet emergent and acute need of participants. Participates in discharge planning for acute and long-term placement. Job Description CACS, Inc. is hiring Part-Time Classroom Assistants for the Head Start and Early Head Start Programs in Ingham County. This is a 28-hour per week position, the rate of pay is $15.00 per hour with no CDA or $15.80 with a CDA. This position requires the ability to work Monday-Thursday and occasional Fridays. The Classroom Assistants encourage children's development in a play-based learning environment. Provides respectful, playful, age-appropriate interactions with children. Assists the teacher and assistant teacher in meeting the social-emotional needs of the children and implements developmentally appropriate experiences. Provides support to the teaching team, families, and bus drivers during the arrival, transitions, and departure of children. Provides professional and reflective communication with co-workers and other departments. Participate in health and safety activities Education and/or certification: • High school diploma or GED., • Coursework in Child Development leading to a CDA preferred., • Preferred work experience in a childcare or related setting., • Assists the teacher in planning and carrying out individualized instruction, for example: guiding physical activities indoor and outdoor; working on self-help skills such as eating and putting on coats; reinforcing learned skills and behaviors with verbal and emotional support; reading and listening to stories; and encouraging relationships with other children. Interacts with children in an effective and positive manner using positive discipline and/or redirection when necessary., • Assists the teacher and assistant teacher in assessing children's development, for example: taking anecdotal notes., • Prepares and participates in mealtime., • Attends and participates in scheduled training, • Assists in maintaining the classroom in a clean, orderly and safe fashion, • Physically interact with children at all times, in an appropriate and safe manner., • May provide childcare for parent activities and/or meetings, • Assists bus driver in the boarding and exiting procedures, monthly bus evacuation drills, release procedures, and monitoring children's behavior while on the bus., • Documents incidents involving children, families, and staff, • Assists with other duties as defined and/or requested, • Adheres to all mandated reporter requirements Employment with CACS Head Start is contingent upon successful completion of a comprehensive background check, including fingerprinting, in accordance with state and federal licensing requirements and Head Start Performance Standards.
