As we launch our new DMH-funded interim housing program, were seeking passionate, experienced professionals to join our growing team. Position Summary: The DMH Case Manager will deliver trauma-informed, client-centered case management to families and Transitional Age Youth (TAY) residing in a 24-hour interim housing facility. This role is crucial to helping clients stabilize, access vital services, and transition into permanent housing.
