JOB PURPOSE The Youth Case Manager is responsible for providing comprehensive, strengths-based case management to youth and young adults. This position collaborates with participants to develop individualized action plans that address academic, career, mental health, and life goals. The Youth Case Manager connects clients to internal and external services and guides them toward sustainable self-reliance through consistent, trauma-informed support. RESPONSIBILITIES • Conduct assessments to identify clients' service needs and barriers hindering sustainable self-reliance., • Develop comprehensive individualized action plans with clients that promote self-reliance, outlining achievable goals and strategies focusing on housing, health, and other essential needs., • Advocate on behalf of clients to secure necessary benefits, entitlements, and services., • Connect clients to internal and external services including mental health support, housing, food, financial aid, education, and workforce programs., • Serve as a liaison between participants and community partners to ensure access to timely, relevant services., • Collaborate with licensed clinicians to coordinate referrals for individual, group, or family therapy as needed., • Establish and maintain an effective and timely referral network to connect clients with necessary resources., • Support clients in developing essential life skills including communication, emotional regulation, budgeting, and conflict resolution., • Work closely with clients to motivate and reinforce independence, self-sufficiency, and self-esteem throughout their journey., • Coordinate with vendors to provide essential items and basic needs for program participants as needed., • Monitor youth mental health and safety, providing support and intervention when needed, including safety planning and connect clients to emergency services when necessary., • Educate participants on budgeting, saving, debt management, and financial literacy to promote long-term stability., • Track educational and vocational milestones, and support youth in navigating systems such as school districts, higher education, or training programs., • Provide immediate support and intervention during client crises., • Maintain up-to-date knowledge of local services, eligibility requirements, and application processes., • Attend trainings and meetings to understand social services in our community., • Conduct case reviews for all clients to assess progress and address emerging needs and to ensure they are receiving necessary assistance and support., • Work collaboratively with healthcare providers, workforce development programs, and social service agencies to provide holistic support for participants., • Mediate issues with clients that threaten their self-sustainability., • Collaborate with the workforce case manager to increase client income., • Stay informed about cultural issues and best practices for working with diverse populations., • Provide services that are sensitive to the cultural, social, and linguistic needs of clients., • Maintain accurate and comprehensive case notes, documenting client status, progress, assessments, service engagement, and any relevant correspondence., • Complete necessary forms and reports following established procedures., • Diligently and accurately record all provided services, referrals, recommendations, case notes, and program documentation into HMIS and Salesforce within 1 business day of contact., • Case notes must be entered within 1 business day of client interaction., • Connect clients with nutrition education support., • Prepare and submit monthly success stories to the supervisor, marketing, and grants manager., • Keep shared spreadsheets up to date., • Case conference with other case managers internally and externally to TJOP to assist shared housing clients in receiving all possible services., • Case conference complex cases/issues with the supervisor and provide creative solutions to challenges that arise., • Any other job duties as assigned.EDUCATION AND EXPERIENCE, • Bachelor's degree in Social Work, Human Services, or a related field, or an equivalent combination of education or relevant work experience, • Minimum of 1 year of experience in the nonprofit sector, • Minimum of 1 year of experience in case management, social services, or community outreach, • Experience working directly with diverse and vulnerable populations, including at-risk individuals, • Relevant certifications in case management and conflict resolution preferred, • Familiarity with housing services, wrap-around programs, and self-sufficiency initiatives, • Demonstrated experience managing a caseload and delivering measurable outcomes related to client stability and self-reliance, • Familiarity with Supplemental Nutrition Assistance Program (SNAP) procedures and requirements preferredKEY COMPETENCIES AND SKILLS, • Mission-Driven: Demonstrates values consistent with The Just One enhance service delivery and client outcomes, • Data Management & Compliance: Proficient in client management systems for case notes, referrals, data entry, and tracking program metrics to ensure compliance, • Resource Navigation: Strong knowledge of community resources and supportive programs, with the ability to provide appropriate referrals and linkagesWORK SCHEDULE AND EXPECTATIONS, • This is a full-time, non-exempt position, requiring approximately 40 hours per week, • Eligible for overtime pay in accordance with the Fair Labor Standards Act (FLSA) and Nevada labor law, • Occasional evenings or weekends may be required for events, outreach, or program deliveryADDITIONAL INFORMATION, • Must attend six mandatory 'Pop Up & Give' events per year, • Must possess valid unrestricted driver's license and reliable transportation, • Must possess or be willing to obtain a valid Food Handlers CardWORK ENVIRONMENT AND PHYSICAL DEMANDS, • Primarily operates in an office environment with regular interaction in community-based settings, • Frequent use of a computer, telephone, and standard office equipment required, • Regular speaking, hearing, and visual acuity necessary for meetings, client interaction, and data entry, • Occasionally required to stand, walk, bend, or lift up to 25 lbs during events, outreach, or community activities, • Must be able to manage multiple priorities in
