The Community Health Liaison will play a vital role in supporting detainees during transition and reentry, coordinating referrals, and strengthening partnerships with community-based organizations. Working closely with Licensed Master Social Workers (LMSWs), nurses, and medical health providers, the Community Health Liaison helps bridge gaps in care and ensure continuity of services post-release.
