As part of our Home-Based Services team, you’ll play a key role in supporting individuals and families who receive care through Minnesota’s PCA/CFSS, Respite, and Homemaking (245D) programs. This position combines compassionate client support with meaningful administrative coordination, helping ensure that each person’s services run smoothly and consistently. We’re looking for a Certified Nursing Assistant (CNA) who enjoys both the people side and the process side of care. Someone who is detail-oriented, organized, and motivated by helping clients maintain the services that allow them to live independently at home. Position type: Full Time Pay range: $18.00-$21.00 / hour with Eligibility for Bonus Incentives Benefits: ACTG offers employer subsidized Medical, employer paid Life and AD&D, in addition to other benefits including 401(k), PTO, Holiday Pay, Dental, and Vision to employees who work 30 or more hours per week. Location: In-Office with some travel to client homes. ACTG is located in a beautiful neighborhood in Northeast Minneapolis close to public transit and freeways, downtown, retail, restaurants, and walking trails. Description of responsibilities: • Serve as the primary liaison between clients, case managers, counties, and the Minnesota Department of Human Services (DHS) for coordination of services., • Facilitate the day-to-day operations of clients receiving support through Minnesota’s PCA/CFSS, Respite, and Homemaking (245D) programs., • Verify client eligibility and obtain necessary service authorizations to maintain program compliance., • Prepare and submit required paperwork and documentation to ensure continuity of care and timely renewals., • Maintain accurate client records, caregiver timecards, and program documentation., • Support clients and families in understanding their home-based services, budgets, and care responsibilities., • Collaborate with the RN to assist with training and competency checks for Personal Care Assistants (PCAs) on ADL tasks, which may include in-home visits to client homes for training., • Monitor quality of services, communicate care concerns, and assist in resolving issues., • Participate in team meetings and contribute to continuous improvement of Home-Based Services operations., • Uphold confidentiality, professionalism, and compliance with HIPAA and DHS standards. Experience: • Certified Nursing Assistant (CNA) certification in good standing (Required), • Current valid driver's license and auto insurance. (Required), • Preferred previous experience in care coordination, home care, or healthcare administration, • Preferred familiarity with insurance verification and service authorization processes, • Preferred Experience maintaining documentation and using electronic care systems Skills: • Strong written and verbal communication (Required), • Attention to detail and accuracy in documentation (Required), • Organizational and time management skills (Required), • Ability to work independently and collaboratively within a multidisciplinary team (Required), • Proficiency in Google Workspace or similar administrative software (Trainable), • Compassionate and professional approach when working with clients and families (Required), • Problem-solving and adaptability in a fast-paced care environment (Preferred) All applicants will receive an acknowledgment confirming receipt of their application.
