Job Description POSITION PURPOSE: The Lead Case Manager is primarily responsible for coordination of services for vulnerable individuals and assisting them in navigating complex service delivery systems. The Lead Case Manager assists persons in gaining access to needed social, medical, educational, and other supports and services. The Case Manager Specialist works on behalf of the person to identify their unique needs and to minimize the impact of the disability on the person's life while assuring continuity of services and support for the person. Excellent interpersonal relationship and leadership skills are required. Essential Functions of the Job- Lead Case Manager · Assists clients in locating and gaining access to necessary services as well as medical, social, educational, or other needed services regardless of the funding source. · Ensures informed choice for all individuals. · Coordinates person centered plan of care using person centered principles that balances what is important to a person with what is important for a person based on their needs and preferences. · Monitors services so the client's health and safety are assured. · Conducts face to face visits every six (6) months or as needed. · Assists in the identification of potential service providers and plans for/arranges services and informs county waiver contact of proposed waiver services to be authorized, for entry into MMIS. · Responds to all Minnesota Adult Abuse Reporting Center (MAARC) reports in a timely manner of receipt of report, documenting all responses and available for review upon request. · Sits and writes and/or types frequently to prepare reports, forms, or information related to overall plan of care. · Functions as a role model, and communicates effectively with clients, families, co-workers, and outside agencies, verbally and in written format; listens accurately and responds appropriately. · Ability to work with clients with a variety of cultural, medical, psychiatric, and physical needs. · Displays positive, supportive, and cooperative conduct. Maintains and fosters cordial relationships and optimal communication. · Adheres to and maintains knowledge of various rules, regulations, and state and county policies regarding the CADI, DD and BI waiver programs, Rule 185 Targeted Case Management; adheres to all policies and procedures/rules and regulations of Phoenix Service Corporation and its governing agencies. · Completes all necessary documentation. Position Summary This position performs behavioral health billing, collection, and accounts receivable procedures; monitors and maintains documentation regarding payer reimbursement regulations and procedures; and communicates with clients and staff regarding billing and collection matters. Job Duties and Responsibilities include: • Review, process, monitor, and resolve services billed to clients and third-party payers – both electronically and in paper form., • Monitor outcome of billing process, research all discrepancies, interact with clients and third-party payers to resolve discrepancies, and refer accounts for outside collection., • Enter record of client and third-party payments, discounts and adjustments into accounts receivable system, while monitoring compliance between Health Plan reimbursement and contract agreements., • Communicate with clients, staff and third-party payers regarding credit policy and other client account billing and payment matters., • Create, update and review billing and reimbursement reports, manuals and other documentation to assure billing is in compliance with governmental and third-party payer rules and regulations., • Monitor aging reports and research and resolve client insurance claims which are denied or uncollected., • Anticipate insurance trends and advise supervisor of changes., • Assist in take-back, overpayments, and refund process to clients and insurance companies., • Works with Senior Billing Specialist monitoring payment compliance for payors., • Works with Senior Billing Specialist on billing errors and projects to get billing corrected and reimbursed., • Performs special follow-up and research for specialized billings. Schedule Monday - Friday, 40 hours/week Qualifications Minimum Qualifications • High School graduation with three years insurance experience, OR, • Post-secondary education equivalent, • Training in office practices, commercial subjects and billing, collections and accounts receivable functions, • Experience working with and/or delivering services to diverse populations., • Demonstrated intermediate experience with Microsoft Excel and basic Word applications required and Electronic Health Record systems and billing knowledge preferred.
