Job Description SUMMARY The Director, Ambulatory Practice Transformation and Quality Performance is responsible for strategic and operational leadership as it relates to ambulatory and specialty value-based care to include the following: quality performance and preventive care; care management, care coordination and transitions;, patient outreach for gap closure, and NCQA to ensure accreditation as a Patient Centered Medical Home (PCMH). Builds a high-performing team which is empowered and accountable to achieve both department and medical group objectives. The Director, Ambulatory Practice Transformation and Quality Performance reports dually, to the Senior Vice President, VHC Health Medical Services, and the Vice President, Medical Affairs, VHC Medical Group with indirect reporting to the Associate Vice President, Quality and Patient Safety. This position is a member of the Medical Groups Executive and Operations Council, and is accountable for meeting financial and operational population health, practice transformation, quality performance goals and standards, as well as, managing to an operational budget. This leader will participate in, and at times lead, VHC Health and VHC Health Medical Group, committees, councils, initiatives and projects, that advance the objectives of VHC Health’s, mission, vision, and anchors/values.
