Within your role you will form a crucial part of the Multi-Disciplinary Team, delivering a high standard of health care to patients living within their own homes, facilitating short term rehabilitation, reablement and recovery in order to avoid unnecessary hospital admissions and enable patients to recover following an acute hospital stay.You will work closely with patients and their families, nursing and therapy colleagues within the team, and will build connections with health and social care partners across the local system. Taking a re-ablement approach the service supports people to return to, and / or, remain in their own home.We are looking for candidates with the ability to motivate individuals, to aid their recovery and maximise independence in order to achieve the goals that matter to them. Roles are available across York and South Hambleton & Ryedale areas.Main duties of the jobYou will undertake duties delegated by the Nursing, Physiotherapy and Occupational Therapy teams, and will be responsible for providing holistic, person centred, compassionate health care to patient’s 18 years and over, supporting rehabilitation, reablement and recovery within the comfort of the patients home.We provide a 7 day service, between the hours of 8am - 8pm; therefore successful candidates will take part in a rota that covers the hours of the service, however we do offer a flexible and varied shift pattern.
