Job Description The Creston Health and Rehab located in Portland, OR has an exciting opportunity available for a Full-time and PRN Certified Occupational Therapy Assistant (COTA) to join their new in-house therapy team! Position Summary: We are seeking a Certified Occupational Therapist Assistant (COTA) to join our in-house therapy team on a full-time basis. The ideal candidate is passionate about patient care, a strong communicator, and committed to helping residents regain independence and improve their quality of life. Key Responsibilities: • Assist the Occupational Therapist in providing high-quality occupational therapy services to residents., • Carry out treatment plans designed to help residents develop or regain daily living and work skills., • Document resident progress, responses, and outcomes in accordance with regulatory guidelines., • Collaborate with the interdisciplinary team to ensure coordinated and comprehensive care., • Educate residents and families on treatment plans and progress., • Maintain a clean and safe treatment environment. You’ll work closely with an OT, deliver hands-on sessions, and document outcomes so the care plan stays clear and effective. Responsibilities • Carry out occupational therapy interventions under OT supervision, • Support patients with self-care tasks, strength, balance, and fine motor goals, • Adapt activities to match a patient’s abilities and safety needs, • Track progress and complete daily documentation accurately, • Communicate with the OT and care team to keep treatment moving forward, • Associate degree from an accredited OTA program, • Active COTA license in Oregon, • NBCOT certification in good standing, • Comfortable working with different patient needs and pacing, • 2 weeks PTO, • Health Insurance
