Job Description DESNOL HEALTHCAR SERVICES, LLC POSITION DESCRIPTION – LEAD STAFF REPORTS TO: PROGRAM MANAGER / DIRECTOR PRIMARY RESPONSIBILITY: The lead staff is a leader, mentor, and coach to the other HHA'S working in the home. The Lead DSP/Staff/caregiver will directly supervise the services delivery to individuals with physical and mental health disabilities. The Lead DSP/Staff/caregiver will ensure all services are delivered according to all agency policies. This position also provides direct care to individuals as needed. This position will need to have frequent detailed communication with staff, clients, and the client’s support team. JOB RESPONSIBILITIES: • Directly supervise service delivery, methods, and accomplishments of outcomes by Direct Support Professionals according to Agency policies and procedures., • Schedule and run medical appointments with the clients., • Train new hires and perform administrative duties such as filling and printing monthly paperwork., • Communicate medical and health concerns on behalf of the clients supported., • Communicate staffing and performance management issues to the Program Manager., • Daily documentation checks: ISPs, Behavior, MARs, Staff Notes, Communication Log, Structured Leisure activities., • Report daily to the Program Manager. Identify missing documentation, client issues, and /or staff issues., • Assist and direct staff to clean the home site: vacuum, mop, bathrooms, clean kitchen/dining areas, and other areas to ensure a home-like, attractive appearance inside and out., • Complete monthly site safety checklist, report repair needs, keep sidewalks and steps free of snow and ice and implement cleaning lists., • Ensure the appropriate menu is posted each week and shop for the house groceries per the menu., • Organize structured group activities daily., • Maintain individual confidentiality and immediately report all client’s rights, safety, or maltreatment issues to the Manager and/or the Director of Services., • Make, provide transportation to, and attend medical, dental, specialist, psychiatric, and psychological appointments., • Inventory hygiene supplies and medications and report to the manager. Contact the pharmacy for refills as needed., • Ensure that all physician and/or nurse treatment orders are in the client’s record/files., • Ensure client clothing is placed in appropriate closets/drawers., • Immediately report all individual rights, safety, and maltreatment issues to the Program Manager and/or the Director of Services., • Cover up when there, • Supervise promoting the safety and well-being of all individuals at home., • Assist the manager with scheduling and documenting quarterly fire drills., • Meet with the Manager once per week for facility audit., • Ensure a safe, harassment, and discrimination-free work environment., • Assist the Manager with various tasks on an as-needed basis., • Required to be in the on-call rotation, taking on-call at as needed at least one time per month. · Using Universal Precautions and appropriate infection control measures • Knowledge of emergency procedures, • Reporting incidents/accidents to the Administrator or Director of Nursing on the day of occurrence, • Participates in care conferences with other healthcare team members., • Maintains confidentiality of all clients, family, and employee information., • Documents as indicated in the clinical record., • Demonstrated awareness of professional boundaries and cultural diversity, • Plans and facilitates activities for clients to enhance socialization., • Assists with personal communication skills as needed., • Promotes a safe and comfortable environment for the client and family while respecting the dignity and privacy of the home environment., • Recognizes and manages own stresses, which may affect work performance., • Participates in in-service educational programs, as required by state regulations., • Performs additional duties as assigned by the Administrator or Director of Nursing., • Adheres to all agency policies. EXPERIENCE/EDUCATION/JOB SKILLS REQUIREMENTS • High school diploma or equivalent, • Associate degree or higher preferred, • Home Health aide course completion preferred., • Satisfactory Three years of experience delivering services to individuals living with disabilities, • Must be 18+ years or older, • Good health status and emotional stability, • The employee must be able to lift and or move up to 50 pounds, • Possess a valid driver’s license and clean driving record., • Operate a computer, calculator, household appliances, and other necessary equipment., • Demonstrate a strong commitment to client service and service excellence The Staff lead is subject to flexible hours depending on coverage and needs of the site. This position may be cross-trained at the other sites and expected to cover shifts if necessary. This position will travel regularly into the community and work in various settings both inside and outside the site and throughout the community. The Staff Lead is subject to frequent interruptions, crisis management, and imposed deadlines. This person is subject to problem-solving activities and has regular contact with individuals, family members, friends, federal, state, or county regulatory personnel, other provider personnel, and the public, who may be hostile or emotionally upset. This position may be subject to verbally aggressive individuals and may be exposed to infectious diseases, odor, and dust. Available shifts and compensation: We have available shifts all days of the week. Compensation is $17.50 - $18.00/hour.
