The Case manager provides ongoing assessments of the client's needs including eligibility for Boston Senior Home Care (BSHC) and other community resources. Furthermore, the Case Manager will develop a service plan based on their assessment of client needs, which directly impacts areas of concern highlighted through the assessment. This position requires the ability to work autonomously, multitask, conduct home visits and family/provider meetings, telephone contact and regular documentation. The Case Manager is responsible for working collaboratively within BSHC as well as externally with client formal and informal supports, vendors, potential referral sources and other community agencies.
