Job Description Position: Assistant Supervisor Department/Program: Aging & Disability Resource Network (ADRN #300) STATUS: ☒Full-time ☐ Part-time ☒Exempt ☐Non-Exempt Reports to: Aging & Disability Resource Network Supervisor Solutions For Care (SFC) serves over 12,000 older adults and those living with disabilities each year in Berwyn, Cicero, and Proviso Townships. We provide direct client service through information and assistance, in-home care, advocacy services, and transitional assistance in hospitals. Primary Function: The ADRN Assistant Supervisor will assist the ADRN Supervisor with the oversight of the program, including maintaining and reporting team productivity to grantors and both the Assistant and Executive Directors. The ADRN Assistant Supervisor will link consumers to available community-based services and appropriate benefit programs. This individual will implement strategic program plans, attend required community, and state meetings, deliver presentations, attend health fairs, and build a network of resources for clients. Weekend and evening hours are occasionally required to attend Health Fairs and Community Events. Qualifications: • Bi-lingual in Spanish/English is preferred., • Minimum qualifications include a BA/BS degree in Human Services, Social Work, Gerontology or related field OR a minimum of four (4) years of experience working with older adults or people with disabilities., • Proficient in Microsoft Office Suite (OUTLOOK, Word, Excel), Acrobat Reader ADOBE Requirements: • Self-driven individual who will take a proactive approach to creating internal efficiencies., • Strong organizational and time management skills., • Adaptability to continually changing work conditions, which may include working weekends on a rotating schedule., • Strong communication skills and the ability to work with diverse populations; Must be comfortable with public speaking and presentations., • Proficiency with Microsoft Office (Word, Excel, and Outlook) to maintain accurate case notes, reports, and record keeping., • Comfortable multi-tasking on the computer using different screens., • Must have a vehicle, a valid driver’s license, and vehicle insurance for local travel to home visits, hospitals, trainings, and meetings., • Criminal background check required. Duties and Responsibilities: • Become knowledgeable of programs, benefits, and services available to seniors and adults with disabilities on a local, State, and federal level., • Complete required initial and annual training by state and other funding sources; maintain required certification., • In the absence of the ADRN Supervisor, the ADRN Assistant Supervisor will assist with supervisory duties, such as addressing staff and clients’ questions and/or concerns as well as responding to staff attendance request., • Collect and collate data from ADRN Advocates and Assistant Supervisors on a weekly basis and complete monthly report to monitor and ensure that monthly production goals are met., • Collaborate with ADRN Supervisor to develop new strategies for departmental and staff growth., • Assist and support all Aging & Disability Resources Network duties and responsibilities as assigned by Program Supervisor, including:, • Assess client needs sensitively and professionally while assisting with applications:, • Connect clients to resources that will meet their needs., • Provide client follow-up to ensure programs/application assistance have been met., • Create Public Awareness through educating the community with presentations and participation in health fairs and community events., • Manage offsite enrollment events that include proper setup of work area, proper display of SFC’s marketing material, including promotion of other SFC services, and proper breakdown of equipment, safeguarding all SFC’s property., • Track, process, and log service units on a weekly basis, while monitoring and meeting production goals., • Participation in team, individual, and agency meetings., • Is an Information Liaison between other SFC departments., • Other duties as assigned by supervisor and senior leadership staff. Schedule: Full time: 8:30am to 4:30 pm, Monday thru Friday, occasional weekends and evening hours to attend community fairs and events. Salary: $45,000.00 TO $53,000. During the first year, six-month and 12-month performance reviews are conducted. Thereafter, performance reviews will be conducted every 12 months. Salary increases are based on performance.
