Job DescriptionDescription: The Resident Nurse (RN)/Licensed Professional Nurse (LPN) is responsible for monitoring the health and wellness of the residents, in respect to overseeing health, nutrition and dietary needs, activities of daily living, medication supervision, and assessing safety concerns of the residents. The RN/LPN responsibilities include, but are not limited to; overall well-being of the residents; communication with all resident support services, physicians, health care agencies, and families; quality assurance; and regulatory compliance. 2nd shift, 32 hours, 3:00 pm to 11:30 pm, required to work every other weekend and alternating holidays. SPECIFIC RESPONSIBILITIES: Documents and monitors the medication management program and distribution of medications to residents by R.C.A.’s. Provides competency training for all resident care assistants in demonstrating competence in Self Administered Medication Management Program (SAMM). • Trains and supervises staff in medication administration and coordinates with pharmacist and other suppliers/vendors for medication delivery and accuracy. Keep all medication sheets updated., • Administers non-injectable medications such as orals, drops, inhalers and topical ointments to residents whose service plans so specify the need for LMA. Maintains written documentation., • Documents and monitors health records and medical information, dementia evaluation history and current records reflecting resident’s physical and mental health., • The RN/LPN will work in collaboration with the Resident Care Director in the planning of the monthly staff schedule., • Monitors and communicates medical issues, program changes, family concerns and staff issues to all other shift managers and resident care team leaders., • Implements quality control/monitoring procedures to ensure that resident care staff performs their routine duties during all shifts., • Coordinates emergency response plan and emergency service for residents, training staff on all shifts for appropriate response to emergencies and company policies., • Monitors and assesses health status of residents, triage emergency situations, and coordinates the delivery of medical or nursing care with certified home health agencies, nurse practitioners, physicians, and rehab services., • Acts as a liaison to families, physicians and health agencies regarding changes and status in resident’s health and medication issues., • Takes an active role in care planning with the Resident Care Director, Executive Director, other departments, outside service providers and families. Directs resident care staff to implement changes and achieve goals of the care plan., • Develop, monitor, implement and evaluate the treatment and care plans for each resident., • Participate in the ongoing monitoring of each participant's health status through direct contact with physicians and family., • Carry out physician's orders for each participant, including vital signs, medications, blood sugar monitoring and treatments. Coordinate these orders with family and/or other health care providers., • Assist, monitor and instruct residents regarding personal hygiene, the use of special aids, accident prevention, ambulation and transferring skills., • Maintain inventory of medications and medical supplies, monitor expiration dates, and order supplies as needed., • Carry out health teaching on an individual and group basis., • In conjunction with Resident Care Director, responsible for documentation of initial health assessment and care plans as well as monthly nursing notes and reassessment forms., • Ability to work outside of regularly scheduled hours. Advance notice is not always possible, but will be given when it is., • Complete incident reports and follow-ups as needed., • Licensed Practical Nurse or Registered Nurse in the Commonwealth of Massachusetts; advanced degree in health services., • 1-2 years experience with geriatric/dementia population., • At least 2 years experience supervising C.N.A.’s or Home Health Aides., • Administers non-injectable medications such as orals, drops, inhalers and topical ointments to resident whose service plans specify the need for LMA and maintains written documentation., • Excellent organizational skills and demonstration of strong clinical and documentation assessment skills., • Knowledge of community support services., • Experience in Assisted Living center preferred.
