Job Description Street Outreach Manager (Program Leadership – Field-Based) Salary Range: $80,000.00 – $90,000.00 annually, depending on experience, qualifications, FLSA: Exempt Location: Hybrid – Field Based (Regional / Multi-County) Schedule: Monday – Friday | 8:30 AM – 5:00 PM (flexible for events and emergency response) Work Structure: Hybrid with frequent field presence and regional oversight The Street Outreach Manager leads field-based outreach operations with a dual focus on generating high-quality member referrals into CalAIM Enhanced Care Management (ECM), Community Supports (CS), and Behavioral Health (BH), while building durable, strategic relationships with community-based organizations (CBOs). This role is accountable for outreach performance, referral pipeline development, and the establishment of trusted partnerships that drive sustained referral flow, coordinated care, and improved member outcomes. What This Role Looks Like (Day-to-Day Reality) • Lead and oversee outreach operations within Santa Clara County, • Spend time in the field supporting outreach staff, partners, and referral development, • Manage outreach performance, referral pipelines, and partnership growth, • Collaborate with ECM, CS, and Behavioral Health teams to ensure alignment and handoffs, • Track performance metrics and adjust strategies in real-time What You’ll Do Referral Generation & Pipeline Development • Lead outreach strategies to identify, engage, and refer ECM-, CS-, and BH-eligible members, • Set clear expectations for outreach staff related to referral targets and productivity, • Monitor referral volume, quality, and conversion rates, • Establish and sustain relationships with shelters, housing providers, street medicine teams, behavioral health providers, and community organizations, • Build strategic referral pipelines that support program growth and member access, • Position Pacific Health Group as a trusted partner within the local ecosystem, • Participate in coalitions, case conferences, and coordinated outreach meetings, • Supervise and coach outreach staff with a focus on relationship-building and referral generation, • Train staff on effective community engagement and partnership development, • Conduct performance reviews tied to outreach productivity and outcomes, • Oversee daily outreach activities across Santa Clara County, • Prioritize high-impact locations and partnerships that drive sustainable referrals, • Support staff in complex field-based engagement and enrollment efforts, • Maintain strong knowledge of ECM eligibility pathways and Community Supports services, • Coordinate closely with care management and operational teams for seamless handoffs, • Serve as the primary contact for outreach-related referral activity (if supporting additional regions or plans), • Participate in performance reviews and partner meetings, • Ensure accurate documentation of outreach, referrals, and partnerships, • Analyze referral trends, partner performance, and outreach effectiveness, • Consistent referral volume and successful conversion into ECM, CS, and BH programs, • Strong, reliable partnerships with key community organizations, • Positive feedback from county and health plan partners, • Outreach staff performance, engagement, and development, • Is a strong leader who can manage field-based teams, • Excels at building strategic partnerships and community relationships, • Is data-driven and performance-focused, • Is proactive, solution-oriented, and highly organized, • Thrives in dynamic, fast-paced, community-based environments, • Is passionate about improving access to care for underserved populations Schedule: Monday – Friday | 8:30 AM – 5:00 PM Work Structure: Primarily field-based with regional oversight responsibilities Location: Hybrid – Field Based This includes locations within: • San Diego County, • Santa Clara County, • San Joaquin County Equal Employment Opportunity Pacific Health Group, along with its divisions, is a proud Equal Opportunity Employer.
