Job DescriptionJob Title: Medical Assistant – Population Health Job Description The Population Health Medical Assistant supports primary care teams across a network of clinics, either remotely or embedded within a primary care office, to improve performance on key population health initiatives. This role focuses on closing quality care gaps, promoting annual wellness and preventive visits, and coordinating in-network referrals for value-based patient populations. The position does not provide direct patient care but uses standing orders under physician licensure to place screening and quality-related orders, conduct chart reviews, and support care coordination and outreach efforts. Responsibilities • Review patient charts regularly to identify screening needs, open quality care gaps, and care coordination requirements for value-based patient populations., • Work under appropriate physician licensure to execute standing orders developed by primary care physicians, placing orders for screenings and other services needed to close quality care gaps., • Perform chart preparation prior to office visits for all value-based patients to identify and document open care gaps and needed preventive services., • Manage in-network referrals for all value-based patients, ensuring referrals are processed accurately and efficiently., • Conduct proactive member outreach to schedule patient visits for campaigns such as annual wellness visits, well-child visits, breast cancer screenings, and colorectal cancer screenings., • Help coordinate follow-up visits and transitions of care for value-based patients to support continuity and quality of care., • Transfer patient data related to quality care gaps from clinic electronic medical records into population health management platforms in a timely and accurate manner., • Work payer care gap reports and frequent flier/high-risk patient reports to identify and address opportunities for intervention and improved outcomes., • Collaborate closely with primary care teams and clinical performance consultants to effectively manage value-based patient populations and support practice performance goals., • Fully disclose relevant training, experience, and credentials to help patients understand the scope of services provided., • Work in a collaborative and cooperative manner with teams in embedded physician offices, central support teams, and their supervisors as appropriate., • Apply a basic understanding of metrics such as average length of stay (ALOS), readmission rates, and gaps in care when reviewing patient data and reports., • Use electronic health records and scheduling systems to support appointment scheduling, documentation, and population health workflows., • Perform outreach to patients to promote wellness visits and other recommended screening appointments, reinforcing the importance of preventive care., • Float to different physician offices within the network as needed to address patient population care gaps and support clinic-based initiatives., • Follow instructions, analyze data, coordinate tasks, and manage workload appropriately to meet business and population health goals., • Perform other duties as assigned by the supervisor in support of population health and quality improvement initiatives.Essential Skills, • At least three years of experience as a Medical Office Referral Coordinator, Medical Assistant, Health Plan Utilization Technician, or Managed Care Organization Utilization Technician., • Diploma from an accredited Medical Assistant program., • Hands-on experience with electronic health records (EHR) and electronic medical records (EMR) systems., • Experience with appointment scheduling within a clinical or ambulatory care setting., • Familiarity with CPT codes, with CPT-2 codes strongly preferred., • Experience working with quality measures such as care gap closures, pre-visit planning, HEDIS measures, and patient outreach., • Proficiency with Microsoft Office applications, including Outlook (email and calendar) and Excel., • Ability to analyze data, interpret reports, and use information to prioritize outreach and close care gaps., • Strong organizational skills with the ability to coordinate multiple tasks and manage workload to meet performance and business goals., • Excellent verbal communication skills, including clear, professional communication with patients, providers, and internal teams., • Ability to follow established protocols, standing orders, and instructions accurately and consistently., • Capability to work both independently and collaboratively within multidisciplinary teams in various clinic environments.Additional Skills & Qualifications, • Certified Medical Assistant (CMA) experience or credentials are highly beneficial., • Experience working within value-based care models or population health programs., • Prior experience supporting referral management and care coordination processes., • Familiarity with population health management platforms and transferring data between EMR systems and reporting tools., • Basic understanding of healthcare performance metrics such as average length of stay (ALOS), readmission rates, and gaps in care., • Comfort working in a floating role, supporting multiple physician offices within a defined territory., • Interest in helping to develop and support new programs within a clinically integrated network., • Demonstrated commitment to improving preventive care, closing gaps in care, and supporting quality improvement initiatives.Work Environment This role is based out of a central hub office and requires regular travel to different provider offices within Maricopa County. The typical weekly schedule includes working from a central office on Mondays, working from clinics within the assigned territory from Tuesday through Thursday, and working either from a clinic or from home on Fridays depending on clinic hours and operational needs. The position involves frequent use of electronic health records, population health management platforms, and standard office software. The work environment is primarily office and clinic-based, with a focus on collaboration with primary care teams, clinical performance consultants, and support staff to develop and sustain population health programs and close care gaps across network clinics. Job Type & Location This is a Contract to Hire position based out of Phoenix, AZ. Pay and Benefits The pay range for this position is $22.00 - $26.00/hr.
