In this role you will be responsible for: • Conducting medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment, • Determining follow-up assessments according to patient needs and best practice guidelines, • Communicating significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility, and other personnel involved with patient care, • Assessing health status by obtaining relevant health and medical history., • Performing a physical exam based on the age and patient history as per the Standard Care Arrangement with a collaborating physician, • Ordering, interpreting, and evaluating laboratory tests and other diagnostic studies to assess patient's clinical problems and healthcare needs, • Performing diagnostic or therapeutic interventions consistent with certification, law, and state medical privileges, • Prescribing and monitoring medications as allowed by prescriptive authority through the appropriate State Board Agency, • Developing a treatment plan in collaboration with the patient and caregivers, • Referring patients to physicians as appropriate for consultation or for specialized health resources and treatment, • Ensuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas, • Providing patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits, • Holding case conferences with families when appropriate to advance the care plan, • Master's or doctoral degree as a Board-Certified Nurse Practitioner, • Current State License as a Nurse Practitioner, • Nursing skills as defined by accepted nursing standards, • Sit, stand, bend, lift, and move intermittently, • Must be able to lift at least 35lbs and bear the weight of an average adult effectively, • Current and valid driver's license, • Reliable transportation and valid automobile insurance coverage, • Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in skilled nursing facility settings, • Demonstrates skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles, • Post-acute or Nurse Practitioner experience preferred, • Adult, Family, or Gerontology Nurse Practitioner certification preferred, • Office Location: 770 Kenmoor Ave, Suite 100, Grand Rapids, MI 49546, • Main Service Area: Grand Rapids, Muskegon, Grand Haven, • Office hours: 8:00 AM – 5:00 PM, Monday through Friday Pay Range$100,000—$120,000 USDBackground ScreeningOptimal Care conducts a background screening upon acceptance of a contingent job offer.
