    Patient Care Technician (Home Dialysis) – PD Home Solution LLC Job Overview Join PD Home Solution LLC as a Patient Care Technician and become part of a specialized, patient-centered team dedicated to delivering high-quality in-home peritoneal dialysis (PD) support services. In this role, you will work closely with patients in their homes, supporting safe and effective dialysis treatments while promoting independence, education, and improved quality of life. Unlike traditional hospital settings, this position emphasizes one-on-one care, clinical excellence, and patient empowerment. Your compassionate approach and technical skillset will play a critical role in helping patients confidently manage their therapy in a comfortable, home-based environment. Key Responsibilities • Provide in-home support for peritoneal dialysis (PD) patients, including cycler setup, troubleshooting, and therapy adherence, • Assist with PD treatment monitoring, including reviewing treatment data, vital signs, and patient-reported outcomes, • Monitor and document vital signs (blood pressure, temperature, pulse, respirations, weight) and report abnormalities promptly, • Perform basic clinical tasks within scope, including specimen collection and observation of access sites, • Identify and escalate signs of complications, including infection, fluid imbalance, or catheter issues, • Maintain accurate and timely documentation of all patient interactions, care provided, and observations, • Follow strict infection control and aseptic techniques, especially during PD-related care, • Provide emotional support and encouragement, helping patients build confidence in managing their home therapy Preferred Skills & Qualifications • Experience or strong interest in peritoneal dialysis (PD) or home dialysis therapies, • Knowledge of aseptic technique and infection prevention (critical for PD success), • Ability to accurately monitor and interpret vital signs and patient status changes, • Familiarity with medical terminology, anatomy, and renal care concepts, • Strong communication skills with the ability to educate and empower patients and families, • Comfortable working independently in a home care setting while maintaining professionalism and accountability, • Detail-oriented with strong documentation and organizational skills, • BLS/CPR certification (required) What Makes This Role Unique • One-on-one patient care in a home setting—not a hospital, • Focus on education, prevention, and long-term patient success, • Opportunity to work with a growing company led by clinical expertise in dialysis care, • Meaningful impact—helping patients maintain independence and avoid hospitalizations Ideal Candidate This role is ideal for a motivated, compassionate individual who values clinical excellence, patient relationships, and autonomy.
