Job Description SBHC Registered Dietician - Caroline Co. Schools Job Summary: Responsible for providing clinical nutrition services, including nutrition assessment, modified diet formulation, self-management training and nutrition intervention to Choptank Community Health System (CCHS) patients in the School Based Health Centers (SBHCs). Ensures patient satisfaction, quality care, regulatory agency compliance, and community outreach and education to support improved health and nutrition. This is an exempt, full-time position in pay grade 6 with the pay range of $55,611.00 - $80,636.00. The SBHC Registered Dietician reports directly to the Community Based Programs Director. Required Skills/Abilities: • Valid Driver’s license, • Bilingual language skills (Spanish/English) a plus, • Working knowledge of automated patient data systems, preferred, • Bachelor's Degree and completion of a Dietetic Internship, • Registered Dietician Nutritionist. Uses parameters such as anthropometric measurements, nutrition-focused physical assessment, and interpretation of laboratory values, • Develops, revises, and individualizes nutrition care plans using medical nutrition therapy based on information from the patient, medical record, family, and health care team members, • Provide personalized nutrition counseling to clients managing diabetes, lipid disorders, hypertension, and obesity, • Provide culturally sensitive nutrition education and guidance to parents/caregivers of pediatric patients, • Complies with regulatory standards, including federal, state, and accrediting agencies, • Completes all required documentation and reports in a timely, professional, and thorough manner, • Communicates effectively with all team members, patients and families through conferences, individual consultation, and community-based programming.
