Job Description Livingston Memorial is looking for a Part Time or Full Time, 24-40 hours a week, Physical Therapy Assistant, to support our Rehab team. Must be able to work 1 weekend a month. Position Summary: This position is responsible for continuity of physical therapy treatment and instruction of home health care patients. Under the direction of a Physical Therapist, to detect, assess, prevent, correct, alleviate and limit physical disability, movement, dysfunction, bodily malfunction and pain from injury, disease and any other bodily conditions, in accordance with accreditation requirements, Title 22, licensure regulations and Livingston’s policies and procedures. What You’ll Do • Performs physical therapy treatment as prescribed by the referring physician under the supervision of a Physical Therapist, and in accordance with the Livingston procedure manual and reviews treatment plan as indicated., • Monitors and progresses patients in use of therapeutic and self-care activities to improve function and increase independence, and reports these findings to the supervising physical therapist., • Instructs family members, caregivers, and Livingston personnel involving them in the treatment program by encouraging them to follow through with those procedures that they can safely perform or supervise., • Communicates with the patient’s physician and the Physical Therapist to keep him/her apprised of the patient’s plan of care, responses, and status and ensures physicians’ orders are followed according to Livingston policy., • Communicates, coordinates, and consults with other interdisciplinary team members regarding the patient’s plan of care and status, including case conferences., • Assists in maintaining a list of current medications., • Demonstrates understanding of Livingston and third party payor or treatment guidelines and documentation required for payment., • Attends case conference meetings and prepares clinical and progress notes as required, submitting documentation in a timely manner., • Participates in the Quality Improvement process as assigned., • Understands and adheres to all licensing and certification regulations pertinent to job., • Participates in a process to maintain good internal and external customer satisfaction., • Meets daily average productivity standard set by Livingston (if full-time employee)., • Responsible for maintaining integrity of the medical record for each patient, whose care they are managing., • Documents skilled care and other patient-related forms, accurately, timely and appropriately for reimbursement, regulatory and compliance mandates., • Performs other related duties as assigned. What We’re Looking For • Valid California Physical Therapy Assistant License., • Valid Basic Life Support certification., • Two years’ successful experience in the healthcare setting. At least one year of community health rehabilitation or home health experience., • Valid Driver License and auto insurance, • Knowledge of computer, proficient with Microsoft Word and Excel., • Good verbal and written communication skills., • High level of attention to details., • Able to work independently, take ownership and responsibility of tasks., • Strong professional ethics, discretion and judgment., • Good organizational skills.
