Job Description SUMMARY: Provides nursing care to assigned Residents in accordance with the policies and core values of Mozaic Senior Life. Practices within the scope of the role of the RN. Responsible for assisting the Salsa in the overall evaluation, implementation, delivery and coordination of Resident care services including assisting in monitoring the medication management program. Reports To: Salsa Titles Supervised: Certified Nursing Assistants (Per Diem) Schedule: Per Diem 7am-3pm & 3pm-11pm shifts available. DUTIES Essential Job Functions: 1.Assists in supervising the medication program including clarifying, transcribing and carrying out physician orders, performing medication management evaluations, administering medication and treatment, and provides teaching to Residents. 2.Performs Resident assessments and implements service care plans. 3.Responds to and provides emergency care within guidelines as needed. 4.Documents accurate information in a timely, concise manner as required per State regulations and the Community. 5.Communicates and collaborates effectively with Residents, families, colleagues and other members of the assisted living team to meet Resident and household needs. 6.Maintains positive relations with Residents, families and physicians. 7.Reports any significant incidents or changes with Resident's needs to Executive Director or Salsa. 8.Supervises the CNA's in implementing, delivering and coordinating service care plans to assure compliance with the quality of care. 9.Assists Residents with Activities of Daily Living as needed. 10.Assists with the training of CNA's and participates in orientation of new staff. 11.Participates in educational programs, in-services, meetings and workshops and utilizes current literature to enhance professional skills and knowledge. 12.Participates in quality improvement programs. 13.Adheres to the policies and procedures of Jewish Senior Services. Maintains confidentiality at all times. 14.Successfully completes the Medication Management training and the Core Competencies for licensed nurses. 15.Works weekends and holidays on a rotating basis according to the needs of the department. 16.All other duties as assigned. LifeVest® is worn by patients at risk for sudden cardiac arrest (SCA), providing protection during their changing condition. It is lightweight and easy to wear, allowing patients to return to their common activities of daily living, while having the peace of mind that they are protected from SCA. This is the perfect opportunity for the health professional with patient care and teaching experience to supplement his or her income. A typical LifeVest® patient is in the hospital awaiting discharge, and the Patient Service Representative sets up the equipment and trains the patient and caregivers on the use and care of the device. They also follow up with patients as needed to assure patient understanding and satisfaction. PSRs retain the flexibility to accept or decline assignments as their schedules dictate. Most assignments can be scheduled during free time – before/after work, and on days off. Responsibilities: • Contact caregivers and family to schedule services, • Willingness to accept assignments which could include daytime, evenings, and/or weekends., • Travel to patient’s homes and health care facilities to provide services, • Train the patient and other caregivers of patient (if applicable) in the use of LifeVest®, • Program LifeVest® according to the prescribing physician’s orders, • Measure the patient and determine correct garment size, • Review with patient, and have patient sign, all necessary paperwork applicable to the service., • Transmit signed copy of the Patient Agreement and WEAR Checklist to ZOLL within 24 hours of the assignment, • Manage device and garment inventory, • Have 1 year patient care experience, • Patient experience must be in a paid professional environment (not family caregiver), • Patient experience must be documented on resume, • Completion of background check, • Florida and Ohio candidates must complete a level 2 screening, fee not paid by ZOLL, • Disclosure of personal NPI number (if applicable), • Valid driver’s license and car insurance and/or valid state ID, • Willingness to pay a $30 annual DME fee which is deducted from a completed Work Order, • Willingness to pay for additional vendor credentialing (i.e. RepTrax) if needed geographically Powered by JazzHR gszBq28Quy
