Provides direct patient care in collaboration with the Pre-Admission Testing Center Provider(s) to complete preoperative education and testing in accordance with current HHC guidelines. Applies the nursing process within the context of the organizational framework for Professional Practice. Functions as a clinically competent/expert. Delegates and supervises work appropriately. Reports to Unit Manager/Director of PATC. Responsibilities • Appropriately assesses and plans patient care by: accurately and reliably obtaining, assessing and interpreting data; identifying and documenting problems and expected outcomes based on a plan of care for the preoperative patient; utilizing scientific knowledge base and appropriate physiological/psychological/social/cultural/spiritual concepts; organizing care and interventions; and identifying appropriate resources in order to meet individual and family health goals., • Using evidence based recommendations, coordinates and implements patient-centered care with variances along the continuum by: contributing to and following plans of the health care team and partnering with patient/caregivers; ensuring documentation and communication amongst caregivers is complete; delegating and ensuring completion of activities in alignment with patient safety; and educating patients/caregivers to achieve established health goals in an effort to provide coordinated seamless care., • Utilizes data to identify actual/potential perioperative risk factors, preadmission services and special needs for initiating referrals and implementing additional interventions as indicated. The Gillespie Center has 15 beds for men and Hoskins' Place for has 4 beds for women in downtown Westport. We work to end clients' homelessness by addressing housing barriers utilizing a Housing First service model approach and connecting clients to appropriate resources. Since the facility is an emergency shelter, the aim is to move clients as quickly as possible to a positive housing situation. In addition, the Case Manager is responsible, along with other staff members, for the smooth operation of the shelter facility and community kitchen. Adherence to chores, overseeing the preparation and clean-up of lunch, needs for supplies, general cleanliness of men's dormitory, dayroom, kitchen and outside areas are all examples of this position's responsibilities. Job Description Community Outreach Manager / Outside Sales - Job Description Position Description: The Community Outreach & Marketing Manager is responsible for promoting our non-medical, in-home care services for seniors and generating new referrals for Assisted Living Services, Inc. as well as our affiliated companies by building and maintaining relationships with company resources in all service areas (nursing homes, rehabilitation centers, assisted living facilities, hospitals, adult daycare, hospice, elder law attorneys, geriatric care managers, faith based organizations, organizations supporting seniors and older adults, etc.). You will support business development by participating in community marketing activities and working to establish strong relationships with new and existing referral sources. Reports to: Director of Sales & Marketing Salary Range: $70,000 - $120,000+ (This includes starting base salary plus potential sales commission bonus’) Hours of Work: Full-time Salaried position. Overtime exempt. Minimum of 40 hours / week. Position Duties: • Establish and nurture professional relationships with the key staff at each of our community facilities, senior care professionals and other senior support referral sources., • Responsible for acquiring and managing key facility accounts to secure referral clients., • Meet with facility contacts regularly to discuss their priorities and how we can assist them., • Participate in all senior care and other networking events at these facilities and in our community to represent and promote our services. This will require attending evening and weekend events., • Respond immediately to all service inquiries from facilities and senior care professionals., • Maintain ongoing communication and a consistent follow-up with all the facilities and senior care professionals regarding potential clients and families who have yet to commit to service., • Work closely with other team members to coordinate and ensure that the facility and referral client’s care is fully compliant with their Services Agreement., • Maintain ongoing communication with the facilities and senior care professionals about each referral client and with our staff responsible for that client., • Consult with Director of Marketing on any quality assurance concerns as required., • Maintain open and effective communication with clients, family members, facility contacts, business owner, colleagues, and caregivers., • Become familiar with resources to assist clients, families, caregivers, and office staff about available resources available to assist seniors in our community., • Hold educational events for clients, families, caregivers, and office staff about available resources available to assist seniors in our community., • Speak with potential clients and consult with them on our homecare service offerings., • Participate in relevant office meetings., • Seek testimonials and encourage online and written reviews from clients/families., • Provide weekly updates of activities to Director of Marketing., • Regularly attend daytime and evening marketing and networking events to enhance the visibility of our agency in the community and generate new relationships and business opportunities., • Trade Shows and Conferences: This position requires attending and assisting with all trade shows and conferences that are agreed upon by the marketing team., • Assist with marketing strategies, budgeting, and event planning by developing various elements essential for the growth of Assisted Living Services, Inc and affiliated companies., • Contribute to company social media, web-based platforms, and email marketing communications to promote the brand and generate potential leads., • Develop and distribute the agency’s quarterly newsletter., • Assist in the completion of special projects as assigned by Director. Position KPI’s: • In – Services/Lunch & Learn Meetings: It should be a goal to have at least (4) In-Services per month (1 per week) in different locations., • Networking Events/Trade Shows/Conferences: There are always networking events, trade shows, and conferences throughout the State. The goal is to attend at least 2 per month., • New Client Acquisitions (From Partner Referrals): The goal is 3 new private pay clients per month from Partner Referrals., • New Client Acquisitions (Total): The goal is 4 new private pay clients per month (1 per week)., • KPI’s can be added/changed/deleted from time to time depending on the needs of the company.

This role coordinates pre-arrival and post-arrival services, facilitates cultural orientation and health promotion, maintains required documentation, and connects clients to community resources that support stability, housing, and successful community integration. Training will be provided for GAP case management requirements and service delivery. R&P Case Manager (Including PC-GAPS Case Management) Essential Job Functions • Learn and comply with applicable requirements of state, local, and federal refugee programs, including Reception & Placement (R&P) and PC-GAPS program., • Become proficient in using multiple database platforms for case management, reporting, and compliance., • Coordinate all pre-arrival requirements, including arrangements for clients’ arrival in Bridgeport., • Provide intensive case management services during the R&P period and support a smooth transition into PC-GAP services following the initial 90-day R&P timeframe., • Support clients as they continue with PC-GAPS services, which will mostly include housing stabilization, service coordination, and ongoing support toward self-sufficiency (training will be provided)., • Schedule and facilitate monthly Cultural Orientation and Refugee Health Promotion sessions., • Maintain compliance with detailed documentation requirements and complete all case-related documentation, paperwork, and reports in a timely manner for federal auditing purposes., • Create and maintain case files for each client, including accurate and up-to-date case notes for all communications, services, and referrals., • Make appropriate and necessary referrals for all core services and additional resources that support community integration and client stability., • Maintain consistent, timely case notes for all services and client interactions., • Attend weekly supervision/consultation meetings with a manager and participate in yearly performance evaluations., • Bachelor’s degree with a concentration in a relevant field (social work, teaching, anthropology, psychology, international studies, women’s studies, public health, or related field)., • At least two (2) years of experience in a client-facing role and/or one (1) year of experience working with vulnerable and/or underserved populations., • Strong organizational, communication, and prioritization skills., • Exceptional interpersonal skills and demonstrated ability to collaborate effectively with colleagues and community partners., • Proficiency in Microsoft Office Suite and experience working with client databases., • Reliable vehicle, valid U.S. driver’s license, and valid auto insurance required., • Ability to maintain a flexible schedule to meet occasional after-hours clients’ needs., • Foreign language skills preferred (French, Tigrinya, Arabic, Swahili, Ukrainian).
