DayBreak empowers adults and caregivers with the tools, resources, and support needed to navigate their health and life options effectively. Working in partnership with Managed Care Plans, we deliver CalAIM Enhanced Care Management (ECM) services designed to address both the clinical and non-clinical needs of Medi-Cal members. Our goal is to reduce hospitalizations, prevent institutionalization, and improve health outcomes and overall well-being. Position Summary: The Clinical Case Manager plays a vital role in delivering ECM services and supporting DayBreak’s mission. This position combines leadership and direct service functions to ensure timely, client-centered care for high-risk and high-acuity members. The Clinical CaseManager will: Serve as the primary contact for referrals, connecting with members within 48 hours. • Conduct initial and ongoing assessments, develop actionable care plans, and guide clients in accessing needed resources., • Coordinate with healthcare providers, caregivers, and social support systems to deliver integrated care and improve outcomes., • Provide leadership to ECM service delivery, including mentoring peers, supporting program operations, and ensuring compliance with CalAIM and DHCS requirements. Participate in organizational activities such as events, trainings, and special projects to strengthen DayBreak’s service network. Core ECM Services: • Outreach and Engagement, • Comprehensive Assessment and Care Management Plan, • Enhanced Coordination of Care, • Health Promotion, • Comprehensive Transitional Care, • Member and Family Supports, • Collaborate with APS, hospitals, EMS teams, and Managed Care Plans to provide timely case management and care transitions., • Conduct comprehensive assessments, identify client needs, and create measurable, client-centered care plans., • Maintain thorough and timely documentation in compliance with HIPAA, Elder Abuse Reporting, and State/Federal regulations., • Participate in case conferencing, team meetings, and quality review processes., • Build and maintain connections with physicians, specialists, caregivers, and support networks to ensure coordinated care., • Intervene in urgent situations by arranging temporary supports such as transportation, food, or emergency resources., • Use motivational interviewing and problem-solving to support behavior change and health promotion., • Engage in outreach to connect potentially eligible individuals to DayBreak’s programs and services., • Support organizational initiatives, including events, training, and cross-program collaboration., • Participate in audits, quality monitoring, and program reporting requirements., • Education/Experience, • Bachelor’s degree in Social Work, Psychology, Counseling, Gerontology, Sociology, or related field (MSW desired); OR 2+ years’ experience working with high-risk adults., • 1–2 years of experience in case management and care planning., • Background in clinical case management and working with clients experiencing crisis or mental health challenges, • Experience providing clinical case management overseeing a large caseload of high needs clients and familiarity with the DSM-5, • Experience in health promotion, medical social work, or chronic condition management (e.g., diabetes)., • Excellent communication skills with a compassionate approach to service delivery (Motivational interviewing experience desired)., • Knowledge of resources in Alameda County., • Strong interviewing, assessment, and care coordination skills., • Understanding of relevant laws and regulations, including HIPAA., • Ability to work effectively with frail, low-income seniors from diverse cultures., • Excellent written, verbal, organizational, and time management skills., • Knowledge and awareness of the history of discrimination in America, particularly as it relates to race and racism, and how this history has led to disparities experienced by marginalized communities such as BIPOC, people with disabilities, LGBTQ+ communities, and others., • Proficiency in accurate documentation and data management., • Current/Active California Driver’s License., • Ability to travel within Alameda County for in-person encounters., • Ability to work in a highly collaborative environment, and be part of a small office team, • Ability to work independently, think ‘out of the box’ and be self-motivated., • Ability to prioritize and manage multiple tasks, and to perform under conditions requiring speed and accuracy, • Sitting: 45 to 1 hour while driving within the community, • Intermittent stooping 5 to 10 minutes, • Intermittent standing within the 8-hour shift, • Intermittent reaching within an 8-hour period, • Intermittent typing within an 8-hour period, • This job does involve some repetitive motion within an 8-hour shift, • This job does involve walking while in the community, • This job does involve intermittent sitting while in the community with clients. DayBreak’s office is located in DT San Leandro. However, this position requires traveling to different locations in Alameda County. Hours and Compensation: This is full time, 100% FTE position. Starting salary $68,000- $78,000 DOE and is competitive with small sized non-profits.
