Job Description Position Description: • Conduct outreach to underserved populations in behavioral/medical hospitals, substance recovery programs, primary care practices, school systems, social service agencies, homeless services, etc. to identify persons in need of behavioral health services with special emphasis with those persons at risk for suicide and facilitate their referral and enrollment for services in CREOKS., • Establish collaborative relationships with the above institutions to develop referral networks as well as to advance the behavioral health equity of under served populations., • Provide peer support as well as screening, referral, and education to persons at risk for suicide., • Provide presentations to community providers and the general public about CREOKS Health Services, suicide prevention, behavioral health needs of underserved populations, and behavioral health equity., • Participate in community meetings and conferences to represent and market CREOKS Health Services, Inc and promote suicide prevention, behavioral health needs, and behavioral health equity of underserved populations to other providers and the general public., • Promote CREOKS' recovery housing within the community; assist connecting eligible persons with CREOKS' recovery housing (sober living); and, support those persons assigned to them., • Support CREOKS clinics and community partners in the development and provision of behavioral health programming supportive of under served populations., • Collect outcome data for the OHANA grant either face to face or by Zoom/phone and ensure that data is reported according to the required timelines., • Facilitate and teach the philosophy to staff and program participants that recovery is possible for everyone., • Assist in attempting to contact and re-engage program participants to treatment., • Support program participants in creating and executing Recovery Action Plans and Wellness Action Plans., • Complete the ODMHSAS’ Wellness Coaching certification and orient staff and program participants to the 8 dimensions of wellness and conduct wellness coaching to support participants in reaching their full potential., • Provide education on health-related issues consistent with their training and experience level., • Advocate on behalf of consumers., • Educate and assist consumers with rights, responsibilities., • Attend weekly staff/treatment team meetings, and Care Coordination meetings as assigned by the Site Director., • Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF and CREOKS policies, • To read and be knowledgeable of all current CREOKS policies and procedures., • Maintain productivity averages of direct service to clientele as set by CREOKS for full time employment., • Turn in billing and associated documentation as mandated by CREOKS policy., • Responsible for necessary interaction with agency program participant families and/or other agencies involved with participants to ensure appropriate therapeutic care., • Responsible for participating in mandatory training such as nonviolent crisis de-escalation and CPR, etc., • Provide services and supports to all program participants within the community (to include a participant’s residence, etc.) consistent with the Peer Recovery Support Specialist’s education, experience, training, • Must be in recovery for at least one (1) year from mental illness and/or co-occurring disorders., • High school diploma or GED., • Prior paid or volunteer work with agencies addressing the needs of underserved populations preferred., • Ability to conduct outreach to different providers and promote CREOKS services., • Ability to provide presentations to the other providers and the community about CREOKS services., • Ability to share story of recovery to individuals and groups., • Knowledgeable of mental illnesses and co-occurring disorders., • Successfully complete the ODMHSAS Peer Recovery Support Provider Training and Test to be credentialed., • Become trained and certified as a Wellness Coach according to ODMHSAS standards., • Successfully complete CTI, MI, Columbia, Mental Health First Aid Training, and suicide prevention training., • Maintain annual training to remain culturally responsive to under served populations., • Complete training serving the homeless population as indicated by supervisor.Company DescriptionCREOKS Health Services is a non-profit organization providing comprehensive health, wellness, and social services.
