YOUR ROLE As a Thyme Care Palliative Care Nurse Practitioner, you will be a critical member of the clinical team caring for our members with serious illness. You will have three primary responsibilities: First, you will see members for specialty palliative care outpatient appointments via video visits as part of an interdisciplinary team consisting of our Community Health Workers, Oncology Nurses, Social Worker, and Palliative Medicine Physician. As one of the first Nurse Practitioners to join the palliative care team, you will be an important part of bringing our model to scale. Second, you will be responsible for the oversight and direction of interdisciplinary case conferences for our members with advanced serious illness, in which individualized care plans for our members are created and monitored by our team of Community Health Workers and Oncology Nurses. You will provide clinical oversight to the case management team and direct clinical care, as needed. Third, you will provide education about palliative care to our team of Community Health Workers and Oncology Nurses. In particular, you will focus your teaching efforts on the outpatient management of cancer-related symptoms, serious illness communication skills, and advance care planning. In addition, you will be integral to the development and implementation of our advance care planning program. This role reports into our Senior Medical Director and can be remote or hybrid based in our New York City or Nashville offices. All patient interactions will be virtual via telephone, video, text, or our proprietary virtual care platform. Within your first three months, you will: • Complete training and are up to speed on Thyme Care's systems, tools, technology, partners, and clinical expectations., • Complete virtual palliative care outpatient visits for our members with the highest degree of clinical rigor. You will work fluidly with our in-house care team and providers to ensure any identified needs are met in follow-up to patient visits., • Be adept at leading interdisciplinary case conferences for members with advanced serious illness, guiding our team of community health workers and oncology nurse navigators in the creation of evidence-based, member-centered care plans that focus on interventions that improve member outcomes., • Become the go-to person for our oncology nurse navigators and community health workers when they are working with a member who has advanced serious illness. Duties & Responsibilities: • Performs comprehensive and problem-focused histories and physical examinations, • Interprets laboratory and diagnostic tests, • Interprets and correlates subjective and objective data to formulate a differential diagnosis and establish a working diagnosis, • Formulates a treatment plan for orthopaedic conditions within their scope of practice, • Implements therapeutic intervention for specific conditions when appropriate, • Exercises judgment on conditions requiring consultation, referral or evaluation by the supervising orthopaedic surgeon or other health care professionals, • Orders, collects and performs appropriate laboratory and diagnostic studies, such as routine laboratory tests, EMG, electrocardiogram and radiologic examinations including arthrogram, ultrasound, CT, MRI and bone scan studies, • Performs compartment pressure measurements, digital blocks and regional anesthesia, • Performs minor outpatient surgical procedures such as tendon repair, wound closure and management, incision and drainage, needle biopsy percutaneous pinning of fractures k-wire and hardware removal, • Performs diagnostic and therapeutic procedures such as aspirations and injections of joints, bursars and cysts., • Performs closed reductions of fractures and dislocations, • Applies braces, casts, splints and other orthopaedic appliances, • Evaluates and clarifies clinical conditions Experience: Orthopedic experience in previous professional roles will be required. Orthopedic fellowship is required if no professional orthopedic experience on CV. Particular interest in the field of Orthopedics is required. Hours: This is a full-time position. Essential Duties and Responsibilities: • Facilitates entry of the client into the health care system by assessing the primary health care needs, including eliciting comprehensive health histories, performing physical examinations, ordering and/or performing pertinent diagnostic tests, analyzing collected data to determine client health status and formulating a problem list, • Provides coordinated and continuous primary health care to clients that spans the health continuum by mutual goal setting and planning delivery of care, • Prescribes a variety of treatment modalities to meet the health and illness needs of clients, • Provides health teaching and counseling that promotes a therapeutic and caring relationship., • Manages and evaluates the plan of care process and outcome and then modifies the plan as needed., • Collaborates with other multi disciplinary professionals and community agencies to provide high quality care, • Coordinates services in conjunction with other team members to meet clients primary health care needs and to ensure the continuity of care, • Participates in quality assurance activities through protocol development and collaborative peer review by: assisting in joint practice protocol development and identifying indicators for quality care; developing joint practice protocols and identifying appropriate evaluative criteria, evaluating clinical standards of care based on the quality assurance process; implements recommended changes and/or improvements to guarantee quality of care based on the evaluation process., • Assumes the responsibility for professional development and contributes to the professional growth of others by: developing in depth knowledge of daily program operations, policies and procedures; acting as a preceptor in the orientation of new health care personnel, interns and volunteers; serving as a consultant in specialized populations to other health care providers and community agencies, • Incorporates and disseminates research findings as a basis for practice and to improve health care outcomes., • Identifies clinical questions to be researched, • Assumes the responsibility for maintaining competence in clinical practice by: engaging in a mentor relationship with mid level providers and physicians to gain clinical expertise; developing and maintaining a client case load as clinical competence is achieved to enhance continuity of care and meet Agency productivity standards; participating in appropriate educational activities in order to maintain and increase competence, • Provides cost effective care that meets Agency productivity requirements by practicing collaboratively with the community health nurses to prevent the duplication of services., • Practices within the legal and ethical guidelines established by the Nurse Practitioner Act, the Code for Nurses, the Social Policy Statement and Specialty Standards of Practice, • Documents comprehensive, accurate and continual data on client records and program reports, • Prepares the appropriate program reports to fulfill funding requirements, • Adheres to professional standards as outlined by governmental bodies, American Nurses Associations (ANA), Nurses Association of College of Obstetrics and Gynecology (NAACOG), the American College of Nurse, • Midwives (ACNM), private funding sources, Agency plans/policies and unit guidelines.
