Position Summary Under the general supervision of the LifeSpring Program Manager, the Case Manager plays a critical role in assisting youth and young adults who are at risk of homelessness or experiencing homelessness. This role combines housing navigation with intensive case management and skill development, ensuring that services are delivered through a youth-driven, whole person, trauma-informed model. The position supports participants in achieving independence, obtaining safe housing, and accessing necessary resources, all while adhering to a strength-based, culturally relevant, trauma-informed, and outcome-based approach. Specific Activities: 1. Client Support and Engagement: • Create a welcoming, supportive environment to engage youth effectively and to build rapport, identify strengths and barriers to participant self-sufficiency, and empower youth to achieve goals and permanent independence., • Provide basic needs assistance, including food, clothing, hygiene products, and access to essential services., • Conduct intakes, eligibility screenings, basic needs assessments, and administer tools like the TAY-VI-SPDAT., • Offer crisis management, safety planning, and emergency support as needed., • Empower youth to make informed decisions by providing options and supporting their autonomy. 2. Job Description Case Manager, Transitional Housing Program Position Summary Under the general supervision of the LifeSpring Program Manager, provides intensive case management and independent living skills development to foster youth and probation youth transitioning into independent living. Assist youth in developing skills, obtaining employment, pursuing educational goals, and developing life plans that will support independence. Services are provided in a client-centered, trauma-informed, and culturally sensitive manner that is consistent with Lifeline's philosophy and mission to build self-sufficiency. Possess knowledge and expertise in relationship to transitional age youth, specifically former foster youth, and the specific issues surrounding this population and demonstrate excellent engagement skills. Provide skill development opportunities for youth including life skills training, meditation, economic literacy, transportation assistance, public benefits advocacy, financial assistance, and tenancy training. Provide supportive services to parenting youth including individualized housing, assistance securing childcare, education and support, maternity/paternity provisions, and specialized services for children. Provides linkage to services within the community to support transition to independence as well as aftercare, including but not limited to, mental health services, educational (college or post-secondary training) and employment support, etc. Provide crisis management and safety planning to the youth in the program. Case management will have a therapeutic component and evidenced based practices should be utilized. Job Description Case Manager, Youth Development Services Position Summary Under the supervision of the Youth Development Program Manager, utilize a team approach to provide comprehensive case management, counseling and educational services to youth on probation and their families participating in the YDS program. Services include, but are not limited to, in-home support, parent training, crisis intervention, counseling, case management, conducting placement assessments, transporting participating youth, and community education and development. The Case Manager is expected to function as part of a multi-disciplinary team while making sound, independent decisions for the benefit of the clients and their families. The Case Manager works in a culturally sensitive manner and abides by the legal and ethical standards of the profession. Due to the nature of the workflow and client needs, the YDS Case Manager must be available to work schedules that include days, evenings, and weekends to best meet needs of the population served. Manages, oversees, organizes, supervises and monitors Head Start and Early Head Start (EHS) and State Blended program options, monitors and ensures center compliance with Title 22 state licensing requirements, Quality Preschool Initiative (QPI) requirements, state and federal performance standards, Head Start/EHS service area plans and policies and procedures. Duties and Responsibilities: • Complete weekly, monthly, quarterly and annual reports, • Maintain accurate resident files in compliance with established management produces and funding agency regulations, • Process applicants and lease units by following the guidelines of the Resident Selection Criteria, • Maintain relationships with local government agencies, such as HUD, TAC, etc., • Conduct regular follow-up inspections on maintenance work, grounds and common areas, • 2-5 years of experience in property management, • Experience with supervising staff members, • Experience with Section 8, • Experience with managing periodic inspections (such as HUD, TCAC and City inspections)., • Excellent communication and interpersonal skills, • Microsoft Word, Excel, Outlook and the general computer skills, • Detail-oriented, • Reliable transportation, • Valid California driver’s license, • Bilingual fluency in Spanish and English, • Yardi program experience, • General knowledge of fair housing, 504 ADA, HUD, and Section 42 regulations, • Certified Professional of Occupancy Certification (CPO)- Section 8 only, • Specialist in Housing Credit Management Designation (SHCM)- Section 42 only, • Registered Apartment Manager (RAM) Certification, • $26/hr, • Paid Holidays, Vacation and Sick Time, • Benefits ( Medical, Dental, Vision, Life Insurance ), • Matching 401k (Employer Match) EOE Company DescriptionSolari Enterprises, Inc. is a full service property management organization specializing in multifamily affordable housing.
