Position Summary Vitae Health is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ overall well-being and adjustment to medical and life changes. Key Responsibilities • Conduct comprehensive psychosocial assessments for residents upon admission and as needed, • Develop, implement, and monitor individualized treatment plans, • Provide individual, group, and/or family therapy sessions, • Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability, • Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns, • Collaborate with the Vitae clinical team, the patient’s attending physician, facility nursing staff, social workers, and rehabilitation teams to coordinate care, • Participate in care plan meetings and interdisciplinary team discussions, • Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations, • Provide crisis intervention and behavioral management strategies when needed, • Educate staff and families on mental health conditions and effective communication strategies, • Master’s degree in Psychology, Counseling, Social Work, or related field, • Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist), • 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings, • Strong clinical assessment, diagnostic, and treatment planning skills, • Knowledge of mental health conditions common in SNF populations (e.g., depression, anxiety, dementia, adjustment disorders), • Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling), • Ability to work collaboratively within an interdisciplinary team, • Excellent communication and interpersonal skills with residents, families, and staff, • Strong organizational, time management, and documentation skills, • Excellent earning potential with competitive base salary plus uncapped bonus opportunities or flexible fee-for-service options, • Consistent, reliable referral flow so you can focus on patient care, not building a caseload, • Comprehensive benefits package including Health, Vision, and Dental insurance, • Generous paid time off: 3 weeks PTO, 6 paid holidays, plus 1 dedicated day for education and professional development, • Full malpractice coverage including tail insurance, no out-of-pocket costs, • Retirement planning support with a 401(k) and company match, • Ongoing professional growth through CEU opportunities and paid licensure renewals, • Supportive, clinician-focused leadership team that prioritizes your success and well-being Vitae Health is an Equal Opportunity Employer and does not discriminate based on race, color, religion, sex, national origin, age, disability, or any other protected status.
