Job Description ROCKFORD HOUSING AUTHORITY Job Description Youth Build Developer/Case Manager SALARY LEVEL: A-10 LEVEL DEPARTMENT: Human Services Programming UNION: ASCME SUPERVISOR: Human Services Manager Under the general supervision of the Human Service Manager or Program designee. PRIMARY RESPONSIBILITIES: The Youth Build Case Manager is responsible for working with participants to ascertain needs and ensure they are enrolled in supportive services to address barriers. The Case Manager will also develop career development, professional goals, and job coaching strategies for participants. DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. • Conduct assessments with prospective participants to determine program eligibility, identify barriers to successful achievement, develop training/service plans with residents to meet established needs, and provide guidance and assistance with their training in cooperation with Comprehensive Community Solutions (CCS)., • Demonstrate strong flexibility and tolerance in dealing with persons from disadvantaged social, economic, and educational backgrounds., • Maintain professionalism, using tact and courtesy to establish and maintain effective relationships with persons contacted during work., • Assist with special projects, grants, marketing, and training programs., • Partner with CCS staff or (YB) partner, community employers, and RHA departments to proactively identify employment opportunities and ensure full compliance with Section 3 requirements, prioritizing economic opportunities for eligible residents and businesses., • Assist with testing and assessments as needed and utilize available resources, assessments, and exploration tools., • Work in strong collaboration with partner programs and participate in community training as necessary., • Build and maintain working relations with community organizations and city, state, county, and federal agencies., • Evaluate each participant's need for assistance with supportive services, employment barriers, transportation, substance(s) abuse, mental health issues, child care, financial aid, etc., and refer participants to the appropriate services., • Develop an Individual Development Plan (IDP) for each participant., • Provide follow-up services to ensure participants are on target with goals identified in their Individual Development Plan., • Make presentations to local clubs and organizations., • Coordinate with the Program Director, GED Teacher, Construction/Safety Instructor, and Job Developer in developing job coaching focused on motivation and career approaches to development., • Collect and input data in the MIS system as required by the grant, monthly, quarterly, and/or annually., • Coordinate, refer, and follow up to ensure participants have access to needed services., • Help with Lifeforce Development Institute, (LDI) lesson preparation, including collecting materials and setting up equipment, • Instruct LDI lessons or lectures with students in small groups, • Maintaining supervision of students in class, • Tracking attendance, grading assignments, and calculating grades, • Assisting with Intake and one-on-one interviews over the 12 weeks, • Collaborating with other Instructors or Program Staff to identify any issues and concerns that recommend team solutions, • Deliver services as written in the Solicitation Grant Applications (SGA) and Statement of Work (SOW) in the grant application., • Valid driver's license, valid automobile insurance, and own vehicle., • Travel as required both within and out of the service area., • Attend professional development training opportunities and participate in conferences and workshops related to job duties., • Other duties may be assigned. EDUCATION AND/OR EXPERIENCE: Associate's degree or completion of 60 college credits and at least 3 years of related work experience working with disadvantaged populations. DESIRED QUALIFICATIONS: Experience in life skills development coaching within a structured social service program, as well as experience working with private and public industries in the community, is desired.
