The Adult Learning Disability Service is excited to bring this opportunity for one experienced and ambitious practitioner (Nurse or AHP) to join our team as a Community Specialist Health Transition and Referral Coordinator. This pivotal role focuses on triaging and screening all referrals for young people with a learning disability aged 17 ½ to 18 into our adult learning disability team in South Essex. • Triage and screen all referrals for young people with a learning disability aged 17 ½ to 18 and coordinate their transition into the adult learning disability team., • Collaborate with mainstream health, education and social care services to develop and implement pathways, provide education and training, and promote reasonable adjustments to improve accessibility for this service user group., • Visit young people in their homes, schools and day‑care settings across Southend, Castlepoint, Rochford, Basildon, Brentwood, Wickford and Thurrock to complete the triaging process (car driving required)., • Support the movement of service users between the wider multidisciplinary team (MDT) using the internal referral pathway., • Lead weekly MDT meetings to prepare and discuss internal referrals, transition referrals and complex cases., • Develop care plans that reflect the needs and wishes of the service user and their family/carers, and agree safe clinical outcomes that address individual health needs., • Provide advice and guidance to young people and families as they approach their 18th birthday., • Receive supervision and support to help you fulfil your potential., • Join an inclusive community and connect with others through engagement events and equality champion networks., • Access mental‑health and wellbeing services, occupational health advice and counselling., • Participate in recognition awards that celebrate staff hard work and dedication., • 27 days holiday, plus bank holidays, rising to 33 days after 10 years’ service., • Excellent pension of up to 14.5% of your pensionable pay., • Staff discounts including Blue Light Card, NHS discount offers and other staff benefits., • £8K relocation package if you move to Essex to join us., • Season ticket loans are interest‑free to cover the cost of travelling to and from work via tram, rail or bus., • Flexible working available from day one for most roles. #J-18808-Ljbffr
