The BronxWorks Homeless Outreach Team (HOT) is a mobile unit that works with homeless individuals on the streets, 24 hours a day, 365 days a year. Since 1995, we have helped street homeless people obtain appropriate services, move to safer environments, and get placed in permanent or long-term transitional housing. The team is composed of outreach workers, case managers and social workers and also partners with psychiatric and medical providers to connect clients to appropriate care. RESPONSIBILITIES • Supervise team of 5 staff (2 Case Managers/3 Outreach Workers) and coordinate team activities focused but not limited to homeless individuals engaged at Lincoln Hospital., • Maintain familiarity with medical and social services and resources available both on-site and in the community, • Provide referrals, care coordination, and direct assistance to support clients applying and/or attending appointments for: Medicaid/Medicare, mental health services, medical care, and substance treatment, • Coordinate and lead case conferences with hospitals/medical providers and social service providers, • Conduct comprehensive needs assessments and psychosocial reports with clients, • Document all interactions with or on behalf of clients, • Provide direct counseling services, as assigned, • Attend regular supervision sessions with direct supervisor, • Conduct field visits to engage and build rapport with chronically homeless individuals, as required, • Drive agency vehicle to and from client locations and appointments, • Complete regular program reports, • Perform additional duties as assigned QUALIFICATIONS • Master’s degree in social work, public health, or a related field. LMSW preferred, • Candidate with a valid driver’s license preferred, but not required, • Experience within a hospital and/or residential setting where medical, psychiatric, and medication monitoring services are provided is preferred, • Ability to engage and build rapport with populations with high support needs, • Strong oral and written communication skills, • Detail-oriented and resourceful with strong time management and organizational skills, • Proficiency in Microsoft Office suite and other standard business technology, • Bilingual/multilingual preferred, but not required PHYSICAL REQUIREMENTS • Ability to use a computer for prolonged periods., • Ability to occasionally lift and/or move up to 10 pounds., • Ability to stand, walk, or sit for long periods of time., • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork., • Ability to bend and retrieve objects and/or documents., • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation., • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work., • TB Test required with the first 120 days of employment.
