Job Description JOB SUMMARY Under supervision, the Mental Health Professional (MHP) is responsible for planning and delivering mental health support services for individuals dealing with mental illness or emotional disturbances. This includes facilitating treatment plans, case management, outreach, and advocacy to improve physical, emotional, and social functioning. The MHP also ensures linkage to resources, promotes recovery-oriented services, and helps clients build the skills necessary for community living. JOB DUTIES • Conduct needs assessments and crisis intervention services (e.g., IM+CANS, LOCUS)., • Plan and implement individual and group therapeutic activities., • Serve as a client’s primary case manager when assigned., • Develop treatment plans under licensed staff guidance., • Link clients to housing and community resources., • Maintain thorough and accurate documentation in compliance with HIPAA and agency policies., • Participate in multidisciplinary team meetings and staff conferences., • Coordinate with external service providers to ensure care continuity., • Advocate for client inclusion in employment, education, and housing services., • Perform transportation of clients as needed., • Mental health services and behavioral health best practices., • Techniques in case management and crisis intervention., • Cultural and socio-economic factors affecting behavioral health., • Rehabilitation, therapeutic, and social service community resources., • Strong communication, both written and verbal., • Ability to perform assessments and develop individualized plans., • High level of empathy and professionalism., • Ability to work independently and collaboratively., • Computer proficiency and record-keeping accuracy., • Must possess a valid driver’s license and meet agency vehicle insurance standards., • Must be able to sit, stand, walk, bend, twist, lift light loads, reach, and operate a computer and vehicle.
