Job DescriptionDescription: • Job Summary: Coordinates intake, evaluation, implementation, and provision of services, record keeping, generation of all needed/mandated noting, reporting, documentation. Reports to Guardianship Program Director. Is instrumental in the provision of care and service to and for Wards appointed to the agency by the Courts or to and for those who voluntarily become Wards of the Agency. Carries case load based on needs of the Program, and based on assessment of the Worker/Ward appropriateness., • Essential responsibilities:, • Takes part in regular and ad hoc meetings and conferences to assess Ward, discuss services, needs, approaches, and efforts. Meets with attorneys regarding planning and needs; attend court sessions as required by the courts, • Reviews, assesses and communicates with members of the Guardianship team, information relating to the Ward and services provided or desired, • Receives bills and other financial documents, reviews, completes request for checks, and submits for approval to the Program Director. Files all copies as needed. Acts as first line contact with vendors, agencies, or others involved with the Ward, • Perform ongoing bio-psychosocial assessments including the areas of physical functioning, psychological/emotional functioning, mental status, activities of daily living, instrumental activities of daily living, social functioning, nutrition, and environment. These assessments help determine the Ward’s needs and are performed in client’s home setting, or other settings, as appropriate., • Develop service plans based on assessment to meet client’s needs & to establish a working relationship., • Communicate with Wards, family members, friends, etc. to generate information to better serve the Ward and the will of the court, • Intervene in person or by other means to assist the Ward in maintaining acceptable/attainable levels of function and independence, • Provide outreach and information sharing among professionals and individuals who may need our services, • Document all activity & maintain case records to meet quality and billing standards., • Maintaining familiarity with benefits, services, providers, and other resources in the community. Seek and coordinate services to the Ward, • Attend clients’ interdisciplinary planning meetings., • Pre-plan client’s funeral., • Make medical decisions for client as need arises and consult doctors and those who work closely with client in order to make the best possible decision for the ward., • Provide education for community facilities and other professionals on guardianship as necessary., • Participate in Guardianship hearings in Probate/County Court., • Work with Adult Protective Services, the Courts, attorneys, etc. to assist in legal process of guardianship., • Prepare and submit annual reports of the court for renewal of guardianship. Assist with physician documentation as necessary, • Document all proceedings and maintain necessary records for court, • Other responsibilities:, • Sensitivity to the service populations’ cultural differences and socioeconomic characteristics, • Ability to establish a respectful relationship with clients, • Ability to work collaboratively with other personnel, inside and outside JFS, • Ability to maintain a helping role, • Ability to set appropriate limits and boundaries, • Within two years of employment attend 40 hour guardianship course, pass exam, take state exam and pass to become registered guardian, • Supervisory responsibilities: No., • Access to confidential information: Yes. Much of the information regarding the Wards and their dealings handled in this position is confidential and is held in confidence, but may be shared on a “need to know” basis, and in compliance with HIPAA., • Employee Interaction: Will interact with Wards, family members, Courts, attorneys, accountants, friends, vendors, staff of various facilities, • Physical demands: Standing and walking, • Working conditions: Extended working hours. Travel in the community. Ability to tolerate new and unfamiliar environments, • Qualifications: Master’s Degree in Social Work or a related field, or several years’ experience in a similar, care management role. Experience in the areas of health, mental health, finance, social services, helpful. Typing skill and familiarity with medical terminology helpful.
