Job Description MPF Federal is seeking a qualified Physician – Internal Medicine to provide outpatient internal medicine services at WHASC, JBSA-Lackland. Services will be performed onsite at the Medical Treatment Facility (MTF). The Internal Medicine Physician will deliver comprehensive, evidence-based primary and internal medicine care to adult patients while supporting readiness, access to care, quality improvement, and clinical training initiatives in accordance with MTF policies and privileging requirements. Job Description The Case Manager is responsible managing an assigned caseload of clients having mental illness or substance abuse use and for assessing client needs, developing, implementing, and reviewing service plans, and working with other community resources in meeting/achieving client service. Assesses, plans, implements, coordinates, monitors, and evaluates options to facilitate the continuum of care including pre-discharge planning with appreciate guardians or family member, follow-up appointments, housing, and transportation arrangements upon discharge. Essential Duties: • Accepts newly assigned cases in which the diagnostic intake has been completed by a licensed provider. Completes case management assessment on all clients entering caseload., • Develops appropriate treatment/service plans with clients as the means for implementing appropriate services and developing effective alliances with clients., • Provides on-going supportive case management functions in accordance with the problems, needs, and the strategies identified within the service plan to help the clients achieve the stated goals and objectives., • Participates as directed in the screening of new service requests within the clinic by means of and agency approved procedure., • Provides face to face reviews with the client on a regular basis to assess the progress made in reaching service goals so that the service plan can be modified as necessary., • Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records., • Access to daily admission report with purpose of effectively meeting new patients to coordinate discharge planning., • Meet with patients/contact patient guardian to complete required activities i.e. obtain collateral information regarding living arrangements, patient request and MD ordered activities., • Educates patients regarding effective allocation of community mental health resources while encouraging provision of high-quality patient care., • Document appropriately gained legal authority to communicate with family members to gain collateral information through ROI., • Notify appropriate person of any patients needing to be court ordered due to inability to obtain consent for treatment., • Complete insurance bridges as appropriate and communicate with insurance case managers regarding updates, incidences, and discharge planning., • Attend Discharge and Treatment Team meetings and all meetings relative to case management/ social functions., • Maintains current knowledge of case management/social services, discharge planning, and stay abreast of hospital programs and services provided., • Completes discharge and aftercare planning packages in advance of discharge date, typically 1-2 days prior to discharge, when discharge is projected., • Performs other duties, as requested. Minimum Qualifications: Education and Experience: • Graduate of an accredited nursing program required; Associates Degree in Nursing (ADN), Bachelor of Science in Nurse (BSN) and/or Master of Nursing (MSN), • Current state licensure as a Registered Nurse, • Current CPR certification (AHA BLS), • To maintain on-call status on an intermittent or regularly scheduled basis., • Ability to satisfactorily communicate in English including reading, writing, speaking, and understanding, • In conjunction with the Vice President, ensures dialysis unit operates in compliance with all ESRD, HCFA, federal, AAMI, OSHA and company policies and procedures, • Responsible for training and development of Skilled Nursing Facility staff while providing clinical oversight to non-licensed support team delegating tasks as appropriate, • Knows and follows policies and procedures under CMS ESRD and state regulations and Rendevor Dialysis, monitors staff compliance of same, • Monitors and assures patients receive adequate medical attention during dialysis treatments and pertaining to dialysis care while a patient of the Skilled Nursing Facility in the dialysis unit, • Conducts comprehensive assessments that include the medical, behavioral, pharmaceutical, and social needs of the patient, identify gaps in care and barriers to good health, • Based on this assessment, and in conjunction with the patient care team, create and implement a care plan that will address identified needs, remove barriers to care, and improve the health of the patient, • Coordinates care by serving as the advocate and resource for the patient, their family, and their provider(s), • Facilitates continuity of care, spanning settings such as the home, hospital, skilled nursing facility, and acute care facility, • Manages patients during periods of transitions of care minimize avoidable readmissions, • Assesses the patient's knowledge of their renal condition and provide education and self-management support, • Provides ongoing reassessment and follow-up to improve patient outcomes, • Reviews and maintains updated policies and procedures on a regular basis, • Develops and monitors quality control measures to assure continuous quality of patient care, • Facilitates Quality Assurance Meetings, • Participates in the IDT and QAPI committee and attend meetings as needed and/or requested, • Identifies and reports concerns, issues, and opportunities for improvement when identified, • Participates in performance improvement projects within licensure, certification, and scope of practice, • Participates in annual QAPI training and other continuing education, as assigned, and required, • Provides suggestions for alternate strategies for indicators not improving, • Assists in building and maintaining a facility/clinic culture of safety and quality, • Reports errors and near misses so that steps can be taken to prevent recurrence, • Reports any negative feedback, complaints, grievances from other staff members, patients, and families, • Uses evidence-based guidelines and proper infection control practices, • Maintains open communication with Skilled Nursing Facility Administrator, Medical Director and Rendevor Dialysis management and staff, • Utilizes resources to improve skills, knowledge, and practice to assure continuous quality of patient care, • Promotes a teamwork approach by offering information, advice, and assistance to all unit staff members in a positive, courteous, and cooperative manner, • Checks email at least weekly and responds appropriately and timely ; uses company-provided email for all work-related email communications, • Maintains regular and predictable attendance and punctuality, • Adheres to all company policies and procedures This job exists in the medical unit of a post-acute facility. This role may include exposure to chemicals and blood-borne pathogens. This role routinely involves the use of both medical and standard office equipment. This position requires the ability to push, pull or lift supplies and products up to 50 pounds. Specific visual abilities required by this job include close vision and ability to adjust focus.
