Job Description This role is located in Kingman, AZ Position Title: Clinical Navigator Department: Oncology Administration Safety Sensitive: YES Reports to: Clinic Manager/Director Exempt Status: NO The Oncology Clinical Navigator promotes patient health and provides patient services to guide patients through the healthcare system by assisting with access issues, coordinating with service providers, and tracking interventions and outcomes. The ideal candidate will have a Bachelors Degree in Health Promotion or have an RN license and will have experience delivering services to groups and individuals in a health education, social or volunteer setting. (fees apply) Discounted membership to on-site gym/wellness center with childcare Career Growth and Development Benefits at KRMC Tuition Reimbursement for full-time employees Scholarships available Student Loan Forgiveness; As a not-for-profit organization, our employees who have qualified student loans, may be eligible for a Public Service Loan Forgiveness program Key Responsibilities · Establishes close relationships with and serves as a primary point of contact for patients. · Assesses and facilitates coordination of multidisciplinary care across care continuum. · Collaborates with multidisciplinary care team and expedite sequence of patient’s workup, active treatment, survivorship, and end of life care where applicable. · Provides communication, advocacy and education in a culturally competent manner. · Ability to clinically triage new patient referrals efficiently, to ensure timely access to care. · Helps patients arrive at scheduled appointments on time and prepared. · Connects patients to community and social support services. · Assesses health literacy and educational needs of patients. · Provides clinical education about care plans, diagnosis, treatments, and processes. · Provides communication and reinforcement of clinical information and serve as a conduit between patient and providers/clinicians to address needs of patients. · Identifies and collaborates with available resources to assist patients to overcome barriers to care. · Works closely with appropriate hospital staff, medical and social service providers, County health, and other community resource providers within Kingman Hospital District communities to identify target populations of patients at high risk for under-utilization of health care services. · Develops appropriate interventions and strategies, considering culture, language, age, gender, etc. and tracks these interventions and outcomes. · Assists uninsured/underinsured patients in accessing available healthcare services in our community, which involves serving as a central point of contact and providing one-on-one assistance to help uninsured/underinsured patients obtain health services based on their financial circumstances and assisting them in completing the appropriate paperwork, scheduling screening appointments, arranging for transportation, etc. · Follows the standards established by the medical staff, radiologists, management team, relevant professional agencies, and regulatory agencies. · Stays current with national best practices and guidelines. · Provides backup coverage to other positions in the clinic qualified to do so. · Attends and represents the organization and/or department at professional conferences, in-service trainings and meetings at the request or with the approval of supervisor. · Conducts outreach and education activities within organization’s communities, which involves formulating relationships within district communities by visiting churches, schools, libraries, etc. · Conducts classes and lectures and working with individuals and groups to promote patient health. · Facilitates the development and administration of a Quality Assurance Program for patient care which guarantees compliance to state and federal regulatory and licensing agencies. This will include monitoring quality metrics and the preparation of quarterly reports which document the quality and compliance of all standards. · Participates in process improvement initiatives. · Documents main navigation services according to program standards. · Performs other duties as assigned. · Collaborates with case manager to facilitate a cancer support group. · Provides qualified patients with survivorship planning. · In collaboration with other oncology navigators, organizes a cancer prevention and a cancer screening event each calendar year. · Assists patients through the screening, diagnostic and treatment process, which involves initiating communication with patients upon learning they have a suspicious or positive oncology finding. · Enhances access to and optimize communication about clinical trials with patients and families. Qualifications Education: Nursing degree or bachelor’s degree in health promotion Certification: Current BLS through the American Heart Association or the American Red Cross Licensure: · Current and valid U.S.A. driver’s license · If RN, current and valid Arizona RN licensure Experience: Experience in delivering services in a nursing, health education, social, or voluntary sector setting with groups and individuals Specialty Area: Oncology Continuing Education: Must complete 12 hours of cancer-related continuing education every calendar year or maintain a current oncology nursing certification including but not limited to: 1. Advanced Oncology Certified Nurse Practitioner (AOCNP), 2. Advanced Oncology Certified Clinical Nurse Specialist (AOCNS), 3. Advanced Oncology Certified Nurse (AOCN), 4. Blood & Marrow Transplant Certified Nurse (BMTCN), 5. Certified Breast Care Nurse (CBCN), 6. Oncology Certified Nurse (OCN), 7. Certified Professional Patient Navigator (CPPN), 8. Oncology Nurse Navigator- Certified Generalist (ONN-CG), 9. Oncology Patient Navigator- Certified Generalist (OPN-CG), 10. Breast Patient Navigator Certification (CPNC), 11.
