JOB SUMMARY: A parenting coordinator assesses clients’ parenting needs and services for on-site parents in residential services. This includes assisting and guiding parents toward meeting DCF goals toward reunification, and coordinating child visitations with DCF, clients, and/or family. This includes facilitating or coordinating parenting groups to assist in building parenting skills to support the parent in developing parenting and co-parenting plans. Position Summary The Behavioral Health Coordinator (BHC) is responsible for providing on-site behavioral health coordination and support for patients enrolled in Psych Health Associates’ Behavioral Health Integration (BHI) program. This role manages an assigned caseload across multiple post-acute facilities, and ensures timely screenings, coordination activities, communication, and escalation of changes in condition. The BHC serves as the primary point of contact at assigned facilities for day-to-day behavioral health coordination. Responsibilities include conducting required monthly behavioral health screenings, monitoring for changes in patient status, communicating with facility staff, and escalating clinically relevant information to the BHCM or psychiatric providers when indicated. This role requires strong organizational skills, professionalism, cultural competence, the ability to work independently in the field, proficiency with electronic health records (EHR) and digital tools, and strict adherence to PHA’s policies, workflows, and documentation standards. Always check on residents who have not come to eat., • Maintain good hygiene and practice proper hand washing technique for infection control., • Assists with meals including serving meals and bussing tables., • Assisting with general room cleaning as needed., • Assists new residents upon admission and orients them to facility, staff, other residents, and routines. • Monitors residents’ personal needs and informs Resident Care Supervisor or Assistant Administrator. • Reports any resident behavioral/physical concerns immediately to the Resident Care Supervisor. • Is constantly aware of cleanliness and appearance of Resident Care areas and assists to clean areas of immediate concern when appropriate. • Completes Resident Observation Reports in a precise and timely manner, placing a copy of such in the proper tray. • Assists to help complete the Resident weight record as required., • Answer the phones as needed., • Assists with proper self-administration of medications technique.
