Care Plan Management: Implements, continuously monitors, and dynamically adjusts the prescribed patient care plan based on clinical findings, patient progress, and team input, requiring critical thinking and timely decision-making. Order Management: Precisely interprets, documents, and executes all authorized physician and nursing orders, including medications, tests, treatments, and procedures. Clinical Observation, Documentation, and Communication: Maintains constant vigilance, systematically assesses the patient, meticulously documents findings and treatments in the EHR, and immediately communicates all significant changes to the physician and healthcare team. Discharge Planning: Proactively assesses discharge needs from admission, collaborating with the patient, family, social work, case management, and physicians to arrange post-acute resources and provide education for a safe transition of care. Licensure / Certification / Registration: • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted., • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
