Job Description Position Title: LPN - Biometric Health Screening/Community Outreach Supervisor: Manager of Patient Experience FLSA Status: Non-Exempt/Hourly Job Category: Full-Time, January - March (potentially seasonal - permanent) POSITION SUMMARY The Licensed Practical Nurse (LPN) works closely with the Manager of Patient Experience to provide education to current and prospective clients via biometric health screening events and programs to promote the Vigilant Health benefit, and increase program utilization and advanced care patient enrollment. MINIMUM POSITION QUALIFICATIONS 1 Education: Licensed Practical Nursing degree plus one (1) year of related experience; appropriate licensing within the state of practice 2 Ability to multi-task, set priorities, and manage time effectively 3 Exceptional phone and verbal communication skills along with active listening 4 Customer focus and adaptability to different customer situations ESSENTIAL DUTIES AND RESPONSIBILITIES The description below is intended to depict the general content and requirements for the performance of this position. It is not to be construed as an exhaustive statement of duties, responsibilities or requirements. 1 Create, project, and execute plans of action as new clinic sites are implemented 2 Review past Community Outreach and Screening Activities to identify strategies to facilitate patient engagement and conversion to active patients 3 Determine type of outreach event (Information only, Screening, VH presentation events, or clinical education) and prepare accordingly 4 Build and collect data on potential clients through screenings for patient acquisition team database 5 Collaborate and support efforts of the Manager of Patient Experience to identify and secure screenings and additional service offerings in an existing market 6 Follow up on screening leads from Manager of Patient Experience within current clinic markets 7 Make contact with clients to obtain information and assess the benefit of participation in outreach events 8 Enter appropriate information into spreadsheets/data bases and scan/upload information into Document Management System 9 Maintain a current contact list of clients and associated partners 10 Assist VP of Operations/Development with health screening and promotion events for current and potential clients 11 Perform clinic duties including, but not limited to phlebotomy, triage, referrals, and other general nursing tasks supporting clinical care 12 Other duties as assigned KNOWLEDGE AND SKILLS 1 Performing for or Working Directly with the Public -- Performing for people or dealing directly with the public. 2 Communicating with Persons Outside Organization -- Communicating with people outside the organization, representing the organization to customers, the public, government, and other external sources.
