Position Title:Care Navigator Reports To:Operations Manager Job Summary:As a Care Navigator, you will guide patients through the enrollment and onboarding process for RPM, CCM, and CHI programs. Your role is crucial in helping patientsparticularly those in underserved communitiesaccess and benefit from these services. You will coordinate patient care,facilitatecommunication between healthcare providers and patients, verify insurance eligibility, andmaintainaccuratepatient records. By ensuringtimelyinterventions and follow-ups, you will contribute to improved patient outcomes and reduced hospital visits. Key Responsibilities: • Assistpatients in enrolling in RPM, CCM, and CHI programs,providingclear guidance throughout the process., • Schedule and manage patient appointments, including enrollments, escalations, follow-ups, and healthcare visits such as M11q and DOH-type appointments., • Collaborate with healthcare teams to ensure seamless coordination and delivery of care across RPM, CCM, and CHI programs., • Ensureaccurateandtimelydata entry into EMR systems (e.g.,AdvancedMD) and other platforms, ensuring compliance with HIPAA regulations., • Monitor and manage patient referrals, leads, and opportunities in Salesforce,maintainingaccuracy and following up as needed., • Address patient inquiries and escalate concerns related to RPM, CCM, and CHI services toappropriate healthcareprofessionals., • Proactively follow up with patients to ensure engagement with their care plans,maintainingregular communication with patients, caregivers, and providers., • Conduct insurance eligibility checks via platforms such as e-PACES, verifying coverage for RPM, CCM, and CHI services., • Enhance provider efficiency by performing pre-appointment cross-checks inChronicCareIQ(CCIQ) and HHA Exchange, ensuring patient charts are verified, updated, or entered accurately., • Maintain effective, professional communication with patients and team members through phone, email, and other correspondence, fostering a supportive and welcoming patient experience., • Oversee device ordering and setup for RPM, ensuring patients receive and understand how to use their devices for effective monitoring. Requirements: • High school diplomarequired;additionaleducation or certification in Medical Administration is preferred., • Previousexperience in a medical receptionist or administrative role, ideally in a healthcare setting., • Proficiencyin using RPM and CCM platforms (e.g.ChronicCareIQ) and EMR systems (e.g.,AdvancedMD)., • Experience withSalesforce formanaging referrals, leads, and patient opportunities., • Familiarity with HHA Exchange for coordinating patient care is preferred., • Bilingual fluency in Mandarin & Cantonese is required., • Strong verbal and written communication skills, with the ability to handle sensitive patient interactions empathetically and professionally., • Excellent organizational and multitasking abilities, with attention to detail in scheduling, data entry, and patient follow-ups., • Proficiencyin telehealth platforms, communication tools (e.g., Zoom), Microsoft Office, and HIPAA compliance in virtual environments., • Strong problem-solving skills and the ability to work effectively in a fast-paced, virtual environment.
