Job DescriptionDescription: Under the direction of the Vice President of Quality Assurance and Clinical Risk Management, the Program Implementation Assistant (PIA) supports the development, implementation, and evaluation of the Center for Justice in Cardiovascular Health across all participating sites. The PIA provides cross-site coordination, administrative support, data tracking, and training assistance to ensure fidelity to program protocols. This role also contributes to program evaluation and implements modifications based on findings, helping the program remain responsive, effective, and equity-driven. While primarily focused on program coordination, the PIA may also function as a Community Health Worker (CHW) when needed, assisting directly with patient outreach, education, health coaching, and referral navigation. Working closely with supervisors, site managers, and program staff, the PIA supports planning, workflow development, training, reporting, and communication of program goals and deliverables. At times, the PIA will also engage with the community, particularly during screenings or group events. Essential Functions, Duties and Responsibilities Program Development and Implementation Support • Reviews and understands program design, deliverables and plans to implement and reach stated goals., • Assist with the rollout and coordination of the Center for Justice in Cardiovascular Health at multiple sites., • Assist supervisor by translating program goals and objectives into action plans, by meeting with the project teams and by getting input from the team to develop best practices., • Support the development of workflows, protocols, and training materials that align with proposal objectives., • Contribute to culturally responsive outreach and education materials., • Support onboarding and training of care team members, including Case Managers, Community Health Workers, Clinical Pharmacists, Nutritionists, Behavioral Health Providers, and Certified Medical Assistants., • Serve in a CHW capacity when needed to support patient engagement, outreach, health coaching, and referral navigation., • Work with program staff, clinical teams, data analysts, and supervisor to identify eligible patients at low, moderate, and high cardiovascular risk., • Assist with patient enrollment, scheduling of baseline risk assessments (blood pressure, cholesterol, lifestyle screenings), and follow-up appointments., • Provide administrative support for scheduling, documentation, and reporting of program activities., • Track outcomes, patient enrollment, and referrals in collaboration with Data Analysts, Data Assistant and supervisor., • Assist with preparation of quarterly reports, dashboards, and materials for internal and external stakeholders., • Participates in staff training sessions and other meetings as required by the agency and/or the funders, as needed., • Responds to issues effectively and in a timely manner., • Coordinate workflows across medical, dental, behavioral health, and enabling services., • Support integration of digital health tools (e.g., self-monitored blood pressure, remote patient monitoring, smartphone-enabled ECGs)., • Facilitate communication among team members and with community partners (e.g., housing, food, transportation, legal services)., • Participate in program evaluation by collecting, organizing, and analyzing program data., • Support the implementation of program modifications based on evaluation findings and quality improvement initiatives., • Assist in collecting and organizing data for QA/PI monitoring and Plan-Do-Study-Act (PDSA) cycles., • Support evaluation activities that inform adjustments to workflows, protocols, and program strategies., • Provide administrative support for CME/CEU-accredited trainings and learning collaboratives., • Provide supervisor with information needed for end of month report and/or participates in program evaluation as requested., • Support dissemination of program outcomes and lessons learned through patient stories and feedback. The requirements below are representative of the knowledge, skill and ability required. • Strong understanding of cultural diversity, health equity, and trauma-informed care., • Bilingual proficiency in English/Spanish strongly preferred., • Knowledge of cardiovascular health, chronic disease prevention, and social drivers of health preferred., • Effective communication, problem-solving, and organizational skills, with the ability to manage multiple priorities across sites., • Proficiency with Microsoft Office, Outlook, and EHR systems (eClinicalWorks preferred)., • Ability to collaborate with patients, families, providers, and community agencies, and to build strong interpersonal relationships., • Demonstrated decision-making, priority setting, and work delegation abilities., • Capacity to work independently or as part of a group, meet strict deadlines, and oversee group activities when needed., • Flexibility to support patient-facing responsibilities in a CHW capacity as program needs require., • Excellent knowledge of available community services and resources., • Commitment to confidentiality and professionalism., • Customer-focused approach in interactions with healthcare practitioners, interdisciplinary teams, community agencies, patients, and families with diverse opinions, values, and cultural ideals., • Ability to learn and apply trauma-informed care principles within the scope of the position., • Bachelor’s degree in Public Health, Health Administration, Social Work, or related field preferred; equivalent combination of education and experience considered., • Minimum of 2 years of experience in program coordination, health education, community health, or clinical support., • Experience in community health centers, public health, or cardiovascular/chronic disease programs preferred.
