Job DescriptionWe are a patient-centered, private internal medicine practice specializing in geriatric care, seeking a compassionate and detail-oriented Medical Receptionist / Care Coordinator. This role is the first point of contact for our patients and plays a key role in supporting smooth clinical operations and continuity of care for an older adult population. The ideal candidate can multitask and is warm, reliable, and comfortable working with elderly patients, families, and caregivers. In addition to front-desk responsibilities, this position includes care coordination tasks to support patient care and follow-up. Key Responsibilities Front Desk & Administrative Duties • Greet patients and visitors in a courteous, professional, and compassionate manner, • Answer and triage incoming phone calls; take accurate messages and route appropriately, • Schedule appointments on multiple provider schedules, • Maintain accurate patient demographic and insurance data in the electronic medical record (EMR), • Manage check-in and check-out processes efficiently, • Assist with medication management and refills, • Assist with coordinating referrals to specialists, imaging centers, and community resources, • Track referral statuses and follow up as needed, • Communicate with patients, families, and caregivers regarding appointments and next steps, • Support care transitions (e.g., hospital discharge follow-up scheduling), • Assist clinical staff with non-clinical care coordination tasks, • Communicate routine test results and clinical messages to patients by phone or portal, as directed by clinical staff, • High school diploma or equivalent required, • Prior experience as a medical receptionist or in a healthcare office preferred, • Familiarity with EMR systems (experience with eclinicalWorks is a plus), • Warm, patient, and respectful communication style, • Strong organizational and multitasking skills, • Attention to detail and accuracy, • Ability to maintain confidentiality and professionalism, • Comfort working with elderly patients, family members, and caregivers, • Small, private practice with a close-knit clinical team, • Opportunity to build long-term relationships with patients and families, • 4 Day work week, • PTO and paid holidays, • Optional medical/vision/dental plans, and 401k options., • Stable daytime schedule (no nights or weekends), • Supportive work environment focused on quality care
