Job Description TITLE: Reunification Support Coach Supervisor DEPARTMENT: DHS/Achieving Reunification Center REPORTS TO: Program Manager PROGRAM OVERVIEW Achieving Reunification Center (ARC), a non-profit entity, has served parents or legal caregivers with children placed in an out of home placement through the Department of Human Services since 2005. The vision of Achieving Reunification Center (ARC) is that all families with children in out-of-home placement have resources and support provided to them to achieve timely reunification. POSITION SUMMARY The Reunification Support Coach Supervisor (RSCS) is responsible for guiding the day to day work of up to five (5) coaches within the RSC Unit. As part of this work, the RSCS is accountable for ensuring that coaches effectively utilize a parent centered coaching model to help parents create goal action plans (GAPS) that directly impact reunification barriers and that the coaches partner with their parents to complete their GAPS. The RSCS conducts regular checks of participant files to monitor GAPs and the delivery of the parent centered coaching model at ARC. Additionally, the RSCS uses data reporting to confirm that parent/caregivers receive high quality support through monthly face-to-face or virtual sessions, as well as follow-ups by telephone and text. The RSCS also uses data reporting to confirm that parents maintain active participation in programming, are connected to needed services and successfully address goals. The RSCS supports coaches to make connections between their work with parents to resolve reunification issues and helping parents to begin the process of strengthening their decision making, persistence and resilience. The RSCS conducts review of reports regarding the progress of parents/caregivers to assigned CUAs and other important parties prior to scheduled Family Court appearances. Job Description Starting Pay Range: $55,000 Position Type: FTE (37.5 hours/week) PURPOSE: Provide strengths-based, person-centered therapy to older adults within the community and over telehealth as needed. The Community Crisis Intervention Team concept is based on the work of PAAN’s predecessor, Philadelphia’s Crisis Intervention Network. This nationally-recognized agency, founded in the mid-1970s by James Mills and Bennie Swans, effectively addressed gang violence, drug abuse and drug trafficking and was credited with dramatically decreasing gang-related deaths in Philadelphia for over a decade. The Community Crisis Intervention Program (CCIP) aims to replicate Crisis Intervention Network’s approach to combat today’s community violence. The AV Intervention Case Manager will focus primary of supporting the following AV program: The Intervention Case Manager (ICM) is responsible for working with clients ages 14-18 who are referred and identified primarily through PDAO. Job Description The Community House Manager (CHM) is responsible for managing group homes for Individuals with developmental disabilities. This professional is responsible for maintaining a safe, supportive, home environment which facilitates the development of independence, empowerment, and self-help skills among Individuals. The CHM is responsible for four core duties; directing care for up to six (6) Individuals residing at assigned homes, ensuring that assigned homes meet the compliance standards, managing allocated funds and resources for assigned homes, and supervising the work of Direct Support Professionals (DSPs) at assigned homes. Other duties may be assigned on an as-need basis. The intent of this description is to provide a representative summary of the major duties and responsibilities performed by incumbents of this job. Incumbents may be requested to perform job-related tasks other than those specifically presented in this description. The CHM position is a full-time non-exempt hourly position. The CHM is required to be present at assigned homes on a scheduled basis. This position works in conjunction with Program Specialists (PS), and reports to a Residential Director (RD). As KenCCID operates its homes on a 24-hour, 365 day a year basis, the CHM will have the opportunity to work overtime hours, if approved by the RD. monitors vehicle log and GPS reports, reports any damage to vehicles, and ensures that all vehicles are clean and gas levels do not drop below a quarter tank, • Collaborates with fiscal department on all work orders or bids for maintenance services Working with Direct Support Professionals (DSPs) • Ensures that DSPs complete assigned job responsibilities pursuant to their job descriptions and the Individual’s ISP, • Serve as role model and mentor for DSPs in terms of attendance, professionalism, and work ethic, • Align DSP performance with KenCCID mission and values, • Works with scheduling coordinator to ensure that DSPs are available to meet staffing ratios for all assigned homes, • Reviews and approves requests for paid time off and collaborates with HR and payroll on any attendance, timesheet, or benefits issues, • Provides site-specific orientation for all new employees stationed at assigned homes, including training on ISPs, • Provides quarterly site-specific refresher orientation for all employees stationed at assigned homes, including training on ISPs, assessment and medical history, • Observes, measures, and evaluates DSP job performance, utilizing supervision and evaluation tools, • Enforces KenCCID’s standards of conduct and discipline and documents issues where these standards are violated;
