Job Description We're hiring dedicated, dependable Licensed Practical Nurses (LPNs) not PRN or occasional work. This role is ideal for Nurses who want steady, meaningful engagement with clients and are ready to provide specialized care, including Medication Administration, Trach Care, Catheter Care, Full Assist ADLs, and Transfers to/from Electric Wheelchairs. Our 24/7 support model ensures you're never alone in the field and always backed by a responsive, reliable office team. The schedule for this position is 8:00 PM - 8:00 AM, Any day Monday through Sunday, making it a great opportunity for nurses seeking a consistent 12-hour shift while making a meaningful difference in a client's life. Key Responsibilities: * Provide Medication Administration as prescribed, monitoring for effectiveness and side effects. * Perform Trach Care safely, following proper procedures and monitoring for complications. * Provide Catheter Care, ensuring cleanliness, proper function, and accurate documentation. * Assist clients with Full ADLs, maintaining dignity and comfort. * Support Mobility and Transfers, including safe use of Electric Wheelchairs. * Monitor clients' health status and promptly report changes to the supervising nurse or healthcare team. * Maintain accurate and timely documentation of all care provided. * Follow individualized care plans to ensure clients' safety, comfort, and well-being. * Communicate effectively with clients, families, and the healthcare team. * Adhere to all infection control, safety, and hygiene protocols. Private duty, home care assistance and/or direct care experience at an assisted living or skilled nursing facility, etc., is preferred, • Strong clinical skills. Self-directing with the ability to work with little direct supervision, • Certified CPR/BLS and negative TB skin test or chest x-ray, • Licensed driver with reliable and insured automobile (proof of insurance required), • Ability to read, write, speak, and understand English and communicate effectively, • Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull.
