Job DescriptionJob Title: Case Manager Homeless Prevention Location: SAMMinistries: 1919 NW Loop 410 Suite 100 Reports to : Manager of Prevention Services FLSA Status: Non-exempt Approved By: Toni Cambria, Director of Prevention, RRH, Street Outreach Approved Date: August 2025 SUMMARY The Homeless Prevention Case Manager is an experienced social service professional providing comprehensive case management services to individuals and families who are currently at-risk of homelessness, to include those with a military background. The desired candidate will assist individuals/ families with Homeless Prevention and Relocation Re-stabilization services. Must be highly organized, excellent at multitasking and demonstrate the ability to work with diverse populations in a fast-paced work environment. Assists families and individuals with obtaining or maintaining housing stability by developing a strong case plan consisting of goals, objectives, and tasks that will help the family/individual reach their maximum capacity. Facilitates access to community services to help the family/individual increase self-sufficiency. Must have previous housing experience and strong interpersonal skills and ability to develop and cultivate relationships with the social service community and other homeless service providers. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: • Conducts intake assessments over the phone with individuals seeking Homeless Prevention and Relocation services to identify those that meet specific program criteria., • Completes all necessary documentation in accordance with funding source and other requirements and completes necessary documents to process payment of rent, mortgage, or utility bills., • Assists in housing location and completes habitability inspections., • Works in collaboration with property management to help resolve any concerns or issues that may arise to help client remain stably housed and maintain positive working relationships with property managers/landlords., • Provides case management services and conducts required follow-up contact with clients who have received assistance., • Uses evidence-based practices in service delivery such as, Motivational Interviewing, Harm Reduction and the Housing First approach., • Provides field based/mobile case management services, as needed., • Assists families/individuals in developing a Housing Stability Plan (HSP)., • Refers families for necessary services and coordinates with other providers to help families achieve goals set in their case plans., • Maintains necessary documentation, reports and statistics., • Enters client data in the HMIS database within timeframe set by the program guidelines., • Attends case staffing meetings to review client progress and plan services., • Meets regularly with Manager of Prevention Services to review program targets and progress., • Demonstrates and engages in practices that are culturally competent and ability to work with diverse populations., • Attends all mandatory departmental meetings and/or training assigned by the Manager of Prevention Services or Director of Rapid Rehousing and Prevention Services, • Assist families/individuals using multiple grants under the Prevention Program, • Other duties may be assigned. SUPERVISORY RESPONSIBILITIES None QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Extensive previous case management experience is required. Candidate must have excellent time management skills and the ability to effectively communicate both orally and in writing. Must have a high level of energy, creativity and flexibility. Experience and knowledge in providing services to oppressed populations and effectively collaborate with multiple human service providers are essential. EDUCATION and/or EXPERIENCE Bachelor’s degree in Human Services, Social Services, Psychology, or related field. Experience working with at-risk populations is a must. LANGUAGE SKILLS English required, however bilingual (English/Spanish) is preferred. MATHEMATICAL SKILLS Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. REASONING ABILITY Must be sensitive to the dynamics associated with homeless individuals/families. Must be able to react swiftly and effectively in crisis situations. Must possess good conflict resolution skills. Logic and ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Job Description POSITION SUMMARY: The Assistant Mental Health Technician (MHT) Manager supports the Mental Health Technician Manager in supervising, training, and coordinating the activities of the MHT team. The Assistant MHT Manager plays a vital role in onboarding new staff, orienting new residents, maintaining staffing coverage, and ensuring high-quality, trauma-informed care within a residential behavioral health treatment center for adolescents. JOB-SPECIFIC RESPONSIBILITIES: Staff Training and Preceptorship • Act as a preceptor for all newly hired Mental Health Technicians and Lead Mental Health Technicians, providing structured orientation and support during onboarding., • Assist with the continued education and ongoing training of all MHT staff under the direction of the MHT Manager. Resident Orientation • Conduct initial orientation for all new residents upon admission, including introducing residents to facility rules, daily schedules, behavioral expectations, and therapeutic milieu structure., • Support special projects, safety initiatives, and residential events as assigned. Supervisory Support • Provide oversight and direction to MHTs and Lead MHTs when the MHT Manager is off-site or unavailable., • Address immediate staffing, resident care, and milieu management needs as they arise. Scheduling and Staffing Coordination • Complete the weekly MHT schedule, considering staff availability, preferences, and program needs., • Coordinate coverage for callouts, unscheduled absences, and PTO to ensure 24/7 staffing coverage. POSITION QUALIFICATIONS: EDUCATION: • High school diploma or equivalent required; Associate’s or Bachelor’s degree in Psychology, Social Services, or related field preferred. EXPERIENCE: • Minimum of 2 years of experience as a Mental Health Technician or similar position in a behavioral health setting, with demonstrated leadership ability preferred., • Knowledge of Texas HHSC licensing standards and Joint Commission Behavioral Health standards required., • Knowledge of trauma-informed care and adolescent behavioral health principles., • Strong understanding of therapeutic milieu management and behavioral intervention techniques.

Read more about THA here: . Texas Health Action is seeking a Bilingual Patient Advocate (Spanish & English) to serve our Kind Clinic location in San Antonio. Advocates are responsible for optimizing the patient experience, helping them navigate the complex healthcare system — from insurance coverage to linkage to medical and supportive social services. Patient Advocates meet one-on-one with patients helping them understand their options for care. Candidates who are members of populations that are disproportionately affected by HIV (including transgender/nonbinary candidates, Black/African-American candidates, candidates affected by incarceration, or candidates who are living with HIV) are especially encouraged to apply. What would you do: • Support status-neutral engagement activities that involve access to HIV biomedical interventions for prevention and care services, • Provide direct care to individuals wishing to engage in services including, but not limited to: conducting intake and assessments for PrEP/PEP and/or gender-affirming care, brief individual and 0067roup interventions sessions, providing sexual health education and counseling, insurance/benefits navigation, community engagement and outreach, and appointment scheduling, • Identify and address psychosocial barriers to accessing care and facilitates linkage to supportive services and relevant programs, • Monitor issues affecting client access, adherence, and/or retention in care, • Maintain up-to-date knowledge of internal and external resources to ensure continuum of care obtained and ameliorates any barriers to services, • Collect and analyze data to identify program progress and effectively manages clinic operations by utilizing quality improvement tools, • Act as a liaison between local agencies with the functions and operations of the program, • Interpret program guidelines, and other policies and procedures for operations, • Participate in program quality improvement and assist in making required adjustments and promotes program innovation helping to establish and implement goals, objectives, policies, procedures, and systems for the clinic, • Maintain a system of confidentiality according to the clinic’s policies and procedures and HIPAA compliance; and, • Perform related duties and fulfills responsibilities as required Based on job duties and requirements, this position is in salary band 40 has an hourly rate of $20.80. Requirements Your previous work life, experience & education must include: • High school diploma or equivalent, • 2+ years of full-time work/volunteer experience in serving diverse populations, including LGBTQIA communities and communities of color (4+ years of part-time work/volunteer experience may be considered in lieu of 2+ years of full-time work/volunteer experience), • Demonstrated experience taking initiative and exercising independent judgment, decision-making, and problem-solving, • Proficiency using computer software applications, such as Microsoft Word, Outlook, and Excel, • Strong written and verbal communication abilities, • Bachelor’s degree related to a human services field, such as social work, psychology, counseling or public health. Two year degrees with licensure and/or four years of relevant work experience will be considered in lieu of a Bachelor’s degree, • 1+ year(s) of experience working in HIV prevention and/or serving people living with HIV, • Knowledge of insurance navigation, • Familiarity and experience using a trauma-informed approach to working with individuals, • Experience and comfort in issues related to substance use, harm reduction, homelessness, mental health, and LGBTQIA communities, • Ability to work harmoniously with diverse groups of individuals, • Experience working with an ethnically, culturally and racially diverse work staff, • Ability to deal effectively with stress relating to the changing organizational, personnel, and healthcare environment, • Ability to perform phlebotomy or a willingness to be trained in phlebotomy, • If selected for this position, proof of COVID-19 vaccination will be requested prior to offer of employment., • Submitting official transcripts, diplomas, certifications and licenses may be required prior to final offer.
