We are hiring a Registered Nurse (RN) to serve as our Department Lead in Integrated Health Care Coordination (IHCC). What You Will Do As the IHCC Department Lead, your responsibilities will include: • Leading the IHCC nursing team and serving as the primary supervisor for LPN Care Coordinators, • Conducting initial and high-risk nursing assessments for waiver clients, • Completing 12 in-person visits per client per month (minimum 1 hour) as required by FSSA guidelines, • Managing care plans, documenting in EVV-compliant systems, and coordinating chronic disease support, • Connecting clients to appropriate health systems, social supports, and community services, • Monitoring medication adherence, prevention efforts, and wellness goals, • Participating in leadership team meetings, performance audits, and continuous improvement planning What Were Looking For • Active, unencumbered Indiana RN license, • CPR and First Aid certification, • At least 12 years of nursing experience in home health, case management, or Medicaid programs, • Working knowledge of Indiana Medicaid Waiver Programs (IHCC, A&D, TBI, SFC), • Experience with EVV documentation and care coordination, • Strong organizational skills, time management, and commitment to underserved populations What We Offer • $41.00 per hour starting wage, • Predictable schedule, • Hybrid flexibility to support work-life balance, • Reimbursement for mileage, • Paid time off and paid holidays, • Paid training, mentorship, and CE opportunities, • Leadership development and advancement tracks, • Supportive team environment committed to compassionate care Ready to Lead with Heart and Excellence?
