This position will be based out of our CCN Headquarters located in Chevy Chase, MD and support our clients in Maryland and DC. Position Overview: The Director of Clinical and High Acuity Services is a member of the Care Advantage team under the supervision of the Senior Vice President of Clinical Operations and the Regional Director of Nursing. The Director of Clinical and High Acuity Services manages clinical operations in conjunction with all services provided. They promote quality service, cooperation between all types of services provided and growth in directed areas. The Director of Clinical and High Acuity Services will be expected to travel to all office locations as directed. The Director of Clinical and High Acuity Services adheres to all Federal, State, Local and Company policies and regulations. Essential Job Responsibilities: • In collaboration with local and company leadership, adequately represent clinical oversight of licensure responsibilities in the DC and MD markets., • Lead a team of RNs/LPNs that complete and oversee the skilled cases and flex care sites., • Instructs assigned care team members on the needs and care plan details, • Reports on the progress of care to parties involved, • Follows up and maintains relationships with clients, always advocating on their behalf, • Establish relationships with community partners and referral sources, • Keeps Clinical Leadership and Operators abreast of patient condition and/or medical changes, • Reviews clinical and caregiver documentation for full compliance, • Take part in interdisciplinary team meetings to ensure the needs of all patients are met, • Completes home visits as needed for coverage/support, • Full understanding and execution of all policies and procedures adopted and approved by the governing body., • Proficiency as it relates to knowledge of the licensing's regulations and subject matter expert on all Personal Care and Nursing Care questions., • Leads the assigned Skilled and Personal Care nursing staff and designated initiatives/departments in delivering excellent patient care to all clients as authorized by the client's physician and health care standards/regulations., • Responsible for nursing recruitment decisions and ensuring that each nurse is actively licensed and abides by the standards for care and provides the highest levels of service possible., • Assists the SVPCO and RDON in creating and updating clinically based policies and actively seek continual improvement in efficiency and accuracy through quality assurance activities., • Ensures clinical and regulatory compliance of the region and that nurses are trained on documentation expectations and being system users, • Analyzes data trends and assists in refining current or implementing new clinical programs that create the best quality of life for our clients, while still maintaining the efficiency needed for growth., • Provides regular training and updates for nursing staff to keep them abreast of techniques, services or regulatory changes that may affect the care provided., • Assists with overseeing all clinical compliance activities and needs for the region and is directly involved in all patient care complaints stemming from clinical or regulatory concerns; works to de-escalate the situation while seeking resolution., • Provides proper discipline activities as warranted; works directly with HR on all serious personal issues and follows the progressive discipline model., • Measures, analyzes, and reports on clinical metrics and key performance indicators., • Acts as the Company representative in external partnerships and relationships., • Supports the clinical department in the event of an audit or regulatory review within assigned region., • Maintain licenses, revalidations, and delegated contracting of all locations and licenses overseen if applicable., • Performing necessary start of care assessments for region as assigned, including as back-up for the assigned team., • Coaching, mentoring, supporting, and extending disciplinary action as required for assigned team through set reviews as well as throughout employment., • Responsible for setting and achieving goals on a quarterly basis., • Develop relationships with the client's care team to include the physician, ensuring that orders are obtained, and care needs are clear with all involved, prior to delivery., • Oversee collective care team to achieve common goals for the client., • Maintain visibility within the community to establish and connect with community partners, develop and build relationships., • Coordinate and take part in strategic meetings to promote increasing success of the department., • Maintain compliance with medical records and documentation of all assigned high acuity clients., • Consult with physicians or community partners to achieve client goals., • Ensure understanding of skilled services needed by all involved with the extension of training as required.
